Statement of Work (SOW) / Performance Work Statement (PWS)

To remove approximately 475 square feet of asbestos-containing black mastic located under non-asbestos floor tile in the former medical records room (Room G26) at the Boise VA Medical Center.

• Removal of asbestos-containing black mastic from the base concrete floor of Room G26.

• Removal of non-asbestos floor tile, which is in poor condition due to past water intrusion events.

• Removal of wooden platforms (Location B) after asbestos abatement begins; Boise VA Medical Center staff will remove platforms at Location A prior to contractor arrival and will remove platforms at Location B post-abatement after cleaning.

• All removed materials must be disposed of in accordance with local, state, and federal laws.

• Pre-Abatement: Submission of a written asbestos abatement plan detailing step-by-step procedures, EPA notification proof, and EPA Asbestos Worker/Supervisor certifications. An optional negative exposure assessment may be submitted.

• Abatement: Full negative pressure enclosure with HEPA filtered air exhausted outside, maintaining a negative pressure of -0.02 column inches of water. At least one decontamination chamber is required. Use of 6-mil poly plastic for enclosure walls. Asbestos removal must follow OSHA 29 CFR 1926.1101, utilizing wet methods and avoiding abrasive techniques like sanding. Waste must be sealed in 6-mil poly bags/drums and labeled as asbestos waste. Waste bags and equipment must be wiped down before exiting the enclosure.

• Visual Clearance: Performed by a 3rd party Industrial Hygienist.

• Final Clearance: Requires Transmission Electron Microscopy (TEM) performed by a 3rd party Industrial Hygienist following NIOSH Method 7400, including aggravated air sampling.

• Teardown: Removal of all materials and equipment after passing aggressive air clearance.

• Location: Boise VA Medical Center, Building 67, Room G26, 500 W Fort St., Boise, ID 83702.

• Period: To be coordinated with the COR. Unforeseen events requiring additional time must be communicated in writing.

• Contractor Personnel: Must possess appropriate asbestos abatement certifications.

• Waste Handling: Must comply with all local, state, and federal laws for handling, transport, and disposal. Hazardous waste manifests must be submitted to the COR.

• Damage Reporting: Any accidental damage to existing building materials or systems not within the scope of work must be reported immediately to the COR.

• Parking: Contractor must communicate parking needs to the COR in the abatement plan. Parking is restricted to assigned or specially reserved areas.

• Restrooms: Public restrooms near the worksite may be utilized.

• Oversight: The COR or VA Industrial Hygienist will monitor the project and reserves the right to "Stop Work" for violations of regulations.

• Security: Access is limited to areas directly associated with the work.

• GFE/GFI: No equipment, materials, services, or information will be furnished by the VA, other than what is outlined in the SOW.

• Submittals: Proof of technical capability (written abatement plan and certifications) and documentation of certified workers/supervisors are required.

• Attachments: Attachment 1 provides a floor plan of the asbestos locations. A parking map will be provided after contract award.
