Solicitation / RFP / RFQ

This is a combined synopsis/solicitation (RFQ) for commercial items, specifically seeking quotations for Smith Medical HOTLINE 3 FLUID WARMER 115V (ICU-HL-390) and HOTLINE 3 TEMP CHECK (ICU HLTA-390). The U.S. Department of Veterans Affairs (VA) requires these items to be delivered to the VA Greater Los Angeles Healthcare System.

• Product: Smith Medical HOTLINE 3 FLUID WARMER 115V (ICU-HL-390) - Qty 20; HOTLINE 3 TEMP CHECK (ICU HLTA-390) - Qty 1.

• Salient Characteristics: The fluid warmer must rapidly warm I.V. fluids to approximately 40°C using disposable sets with "hands-off" gas venting. Specific technical specifications for voltage, frequency, current, and temperature set points are provided. Offerors must demonstrate that their proposed items (brand name or equal) meet all salient characteristics.

• New OEM Items: Only new Original Equipment Manufacturer (OEM) items are acceptable; no used, refurbished, remanufactured, or gray market items will be considered.

• Certificate of Compliance: A completed and signed Certificate of Compliance must be submitted with the offer.
