Scope of Work
This RFQ seeks various specialized surgical tables, including those with Radiolucent Imaging Tops, Spinal Surgery Tops, and a LEVO STANDARD PACKAGE, along with a range of accessories such as adapters, arm boards, positioners, sockets, carts, and kits. All products must meet stringent salient characteristics, including hospital-grade quality, interchangeable tops, C-arm/O-arm compatibility, viscoelastic medical pads, and the ability to position bariatric patients up to 650 lbs. Spinal surgery tops require specific support systems. Only new, non-remanufactured, and non-"gray market" supplies covered by a manufacturer's warranty are acceptable.
Contract Details
This is a Firm-Fixed-Price contract. Offers for "equal to" products must clearly demonstrate how they meet all specified salient characteristics. An Authorized Distributor Letter (ADL) from the manufacturer is required for all suppliers, distributors, and resellers. Compliance with the Buy American Act and Trade Agreements Act must be documented in the quote.
Submission & Evaluation
Quotes, limited to a maximum of 15 pages (excluding the pricing schedule), must be submitted via email to HANAN.MCCULLICK@VA.GOV. Award will be made to the responsible offeror whose quote is most advantageous to the Government, based on technical acceptability (meeting all salient characteristics) and lowest price. Discussions may be held if deemed necessary. Offerors must be registered in SAM prior to award and maintain registration throughout performance.
