Statement of Work (SOW)

To procure and install new mirrors and shelving units in the Restrooms and Patient Rooms of Building 4, Units in Building 4; Rehab and Dementia wards at the VA Boston Healthcare System's Brockton campus. The goal is to create a "home-like, non-institutional, and patient centered environment" that meets strict design guidelines for patient safety.

• Remove and dispose of existing damaged mirrors (2) and paper towel dispensers (16).

• Supply, deliver, and install 70 new "Essential" capsule-shaped framed mirrors (18" x 36", Polished Chrome or Nickel finish).

• Supply, deliver, and install 70 "SSM Shelving Units - Bathroom Solid Surface Wall Shelf" (12"x12"x5", solid surface, glossy or matte finish).

• Installation services include design, removal/disposal of existing furniture, and installation of new items.

• Coordination for delivery via the shared loading dock (South side off Vestibule B012) is required, as the service elevator is out of order.

• Location: VA Boston Healthcare Systems Brockton campus, 940 Belmont Street, Brockton, MA 02301, Building 4-C Units for First and Second Floors.

• Delivery Timeframe: No later than 14 weeks following award of contract, with specific dates to be mutually agreed upon.

• FF&E to be selected from manufacturer's standard offerings, with samples submitted with the proposal.

• Shop drawings, product data, and material samples are required.

• Installation must follow attached sketches and diagrams.

• "Or equal" products must include manufacturer's product brochure.

• Dual Solar Window Shades (Quantity: 104) are also specified, with detailed requirements for dimensions, manufacturer, type, operator, fabric, and fascia. These are to be installed in windows that may be mulled together.

Solicitation / RFP / RFQ

To procure and install new mirrors and shelving units in the Restrooms and Patient Rooms of Building 4, Units C (First and Second Floors) at the VA Boston Healthcare System's Brockton campus. This is part of ongoing renovations to create a "home-like, non-institutional, and patient centered environment" that meets strict design guidelines for patient safety.

• Remove and dispose of existing damaged mirrors (2).

• Supply, deliver, and install new mirrors and shelving units.

• Remove and relocate (16) paper towel dispensers.

• The contractor will be responsible for design, removal/disposal of existing furniture, and installation services.

• Mirrors: 18" x 36" capsule shaped framed mirrors, finish: Polished Chrome or Nickel.

• Shelving Units: 12"x12"x5" solid surface wall shelves, with glossy or matte surface polishing.

• Dual Solar Window Shades: Qty. 104, 54" high x 48" wide (+/-), Draper Flex or approved equal, with specific fabric requirements (Room Darkening and 5% Open Weave) and ivory fascia.

• FF&E: Must be selected from manufacturer's standard offerings and meet minimum requirements for solid surface, powder coated surfaces, painted metal, trim, hardware, and/or fabric samples.

• Shop Drawings: Complete shop and installation drawings, including dimensions, details, and accessory items.

• Product Data: Catalog and model numbers for all components.

• Samples: Provide samples of all materials, fabrics, finishes, and colors as requested.
