This document is an Excel spreadsheet containing a price sheet for custodial services. Bidders are instructed to insert only the UNIT PRICE for each line item. All line items must be priced in accordance with the Performance Work Statement (PWS).

• The spreadsheet is divided into sheets for Ordering Year 1 through Ordering Year 5 (June 1, 2026 - May 31, 2031).
• It lists 18 CLINs (Contract Line Item Numbers) detailing various custodial services, including different levels of facility cleaning, restroom/locker room cleaning, and floor cleaning across different areas (e.g., general facilities, fitness centers, youth centers, CDC).
• Each CLIN includes columns for the service description, ordering period occurrence, unit of issue, estimated square footage per occurrence, unit price (to be filled by bidder), and ordering period price.
• The document specifies that it is a performance-based acquisition and a combination Firm Fixed Price and Requirements acquisition. CLINs marked with "**" are Requirements CLINs, which are not guaranteed for any minimum quantity.
• Bidders are instructed not to price the "NON-PERSONAL SERVICES" line item.

This pricing sheet is critical for bidders to understand the cost structure and to submit their proposals. Bidders must carefully review the PWS (referenced in appendices A through H) and accurately input unit prices for each CLIN. The "Total" fields in each sheet are currently populated with 0.0, indicating that the bidder must provide all pricing information.
