This document provides the government's responses to vendor questions (Requests for Information - RFIs) regarding Solicitation No. W912L3-26-Q-4001 for Project Title Technician Surveillance Exams.

• Government Furnished Property: The government will provide workspace, examination rooms, tables/chairs, and electricity.
• Operating Hours: Required daily operating hours are 0700 to 1630 MST.
• Exam Quantities: Heavy metal panels are required only for selected personnel, not all 70 employees. The quantity '1' refers to one employee per panel.
• Performance Period: All required examinations are expected to be completed in a single mobilization period. For this year, it will be August; follow-on years will be April/May.
• Scheduling: The government will coordinate and schedule all employee appointments.
• Respirator Fit Testing: Quantitative fit testing is required. The government needs to be informed of the make/model/type of respirators currently in use.
• Performance Location: Services will be conducted at the Army Aviation Support Facility (AASF) located on F.E. Warren Air Force Base.
• Workspace: The AASF facility will provide space for contractor operations, including a waiting/check-in area, dedicated respirator fit testing area, private examination rooms, examination tables, administrative workspace, and secure records storage. Electrical/internet access will also be provided.
• Incumbent Contract: The previous contract number for this requirement was W912L3-21-P-0005.

Statement of Work (SOW) / Performance Work Statement (PWS)

To provide medical surveillance physicals for approximately 70 Wyoming Army National Guard Federal Technicians.

• Provide all necessary personnel, equipment, tools, materials, supervision, and quality control.

• Perform medical surveillance exams as defined in the PWS.

• Specific services per individual will be identified by the Wyoming Army National Guard Safety and Occupational Health Office.

• Exams are required annually in accordance with DoD Instruction 6055.5, DoD Instruction 6055.5-M, and OSHA 29 CFR 1910.

• Services include multiple tests described throughout the document.

• Period of Performance: June 1, 2026, to May 31, 2031.

• Place of Performance: Requires guided access to F.E. Warren Air Force Base in Cheyenne, WY.

• Contractor must develop and maintain an effective Quality Control Plan (QCP) and submit it within 10 days of award.

• Contractor and subcontractors must comply with adjudication standards using the National Crime Information Center Interstate Identification Index (NCIC-III) and Terrorist Screening Database (TSDB).

• Contractor shall furnish all required items except for Government-Furnished items specified in Paragraph 3.0.

• Government will evaluate contractor performance based on FAR 52.246-4.

Statement of Work (SOW) / Performance Work Statement (PWS)

To provide job-related medical surveillance examinations and occupational health services to approximately 70 Wyoming Army National Guard full-time support and temporary personnel who are potentially exposed to health hazards or require specific physical fitness standards.

• Medical Surveillance Examinations/Occupational Health Services:

  • Physical Examination/Assessment/Consultation by a Board Certified Occupational Health Physician (Preferred) or Physician's Assistant.
  • Examination to include work/health history, occupational exposure, and relevant medical systems.
  • Submission of a written summary of employee's health status, recommendations for limitations/restrictions, and follow-up to the employer.
  • Approval required from the Wyoming Army National Guard COR for exams/testing outside the contract scope.

• Medical Record Keeping:

  • Ensuring individual medical records are available for review.
  • Employees to complete supplemental forms (e.g., consent, Privacy Act Statement).
  • Minimum identification of records by employee name, social security number, and gender.

• Exams: Includes estimated numbers for Physical Exam, Vision Exam, Chem 20 Lab Tests, Blood Lead/ZPP/Cadmium/CHRO (Heavy Metals), Pulmonary Function Test, Chest X-ray, and FIT Testing.

  • Specific requirements for Physical Exam (vitals, job function analysis, dermatological exams).
  • Specific requirements for Vision Exam (acuity, phorias, depth perception, color perception, peripheral vision).
  • Detailed requirements for Pulmonary Function Tests (PFTs), including procedures, acceptable results, and documentation.
  • Detailed requirements for CHEM 20 Lab Tests (e.g., CBC, Liver Function Test, Urinalysis, Blood Glucose).
  • Detailed requirements for Blood Lead, ZPP, Cadmium, CHRO (Heavy Metals) tests.

• Contractor Manpower Reporting Application (CMRA):

  • Reporting all contractor labor hours via a secure data collection site.
  • Reporting inputs for labor executed during each Government fiscal year (FY) by October 31st of each calendar year.

• Technical Exhibit 1 (PRS):

  • Performance Objective: Provide a Board Certified Occupational Health Physician (Preferred) or Physician's Assistant.
  • Performance Standard: Adherence to military technician schedule (0700 MST start time), with completion based on the last patient seen.
  • Acceptable Quality Levels (AQL): Deviation from COHP or PA requires clearance; acceptable 5-minute late start times.
  • Surveillance Method: Provide licensure; COR surveillance.

• Deliverables Schedule:

  • Written summary of employee health status, recommendations, follow-up: within 15 working days of exam completion.
  • Respiratory clearance documentation: at time of visit.
  • Copies of test results and recommendations: within 20 calendar days after exam completion.
