This document is the Indian Health Service (IHS) Buy Indian Act Indian Economic Enterprise (IEE) Representation Form. It serves as a self-certification for offerors to confirm they meet the definition of an "Indian Economic Enterprise" as defined by HHSAR 326.601, in response to a Sources Sought Notice, Request for Information (RFI), or Solicitation.

• Eligibility Criteria: Offerors must meet the definition of an "Indian Economic Enterprise" at the time of offer, contract award, and throughout the contract performance period. Failure to meet these requirements necessitates immediate written notification to the Contracting Officer.
• Representation: Offerors must indicate whether they do or do not meet the definition of an "Indian Economic Enterprise" with "Indian Ownership" under the Buy Indian Act.
• Required Information: The form requires the Offeror to provide:
  • Name of Federally Recognized Tribal Entity (if applicable)
  • Name of the 51% owner of IEE Firm
  • Business Name
  • Certifying Signature
  • Unique Entity ID (UEI)
  • Print Name
• Legal Implications: False or misleading information submitted is a violation of law, punishable under 18 U.S.C. 1001. False claims during contract performance are subject to penalties under 31 U.S.C. 3729-3731 and 18 U.S.C. 287.
• Additional Requirements: Successful Offerors must also be registered with the System of Award Management (SAM).

This form is critical for any bidder seeking to be considered for contracts under the Buy Indian Act. It requires self-certification of eligibility and must be completed accurately. Contracting Officers may require documentation to verify eligibility, and awards may be subject to protest if eligibility is in question. Registration with SAM is also a prerequisite.

Solicitation / RFP / RFQ

This solicitation is for the design, engineering, furnishing, installation, configuration, integration, testing, and commissioning of a Lenel OnGuard-compatible Physical Access Control System (PACS) and Intrusion Detection System (IDS) at the Inscription House Health Center (IHHC) in Tonalea, Arizona. The objective is to enhance facility security, regulatory compliance, and life safety by implementing a reliable and secure access control and intrusion detection system.

• Provide and install a Lenel OnGuard-compatible access control system supporting approximately forty (40) interior and exterior controlled openings.

• Install a UL-listed intrusion detection system protecting the Pharmacy and designated controlled substance storage areas.

• Provide a dedicated workstation and licensing for IHHC Security for local monitoring and administration.

• Integrate lockdown functionality.

• Program user access levels, time zones, and alarm escalation procedures.

• Conduct full system testing and commissioning.

• Provide training for IHHC Security and IT personnel.

• Deliver complete documentation, including as-built drawings, system configuration documentation, and device schedules.

• Software licenses and configuration backups.

• Minimum one-year parts and labor warranty.

• Period of Performance: 150 business days from contract award.

• Work hours: Monday-Friday, 8:00 am - 5:00 pm, excluding Federal Holidays.

• This acquisition is set aside for Indian-Owned, Small Business Economic Enterprise (ISBEE).

• Proposals must be submitted in PDF format, limited to 15 pages.

• The award will be made to the responsible Offeror whose proposal is the Lowest Price Technically Acceptable (LPTA).

• Invoices must be submitted through the Department of Treasury Invoice Processing Platform (IPP).

• This acquisition is set aside for Indian-Owned, Small Business Economic Enterprise (ISBEE).

• The solicitation includes provisions related to the Buy Indian Act and requires representation of an "Indian Economic Enterprise."
