Statement of Work (SOW) / Performance Work Statement (PWS)

To repair deteriorated asphalt roads at Muñiz Air National Guard Base (ANGB), Puerto Rico, to support the Base Mission. This includes subbase, base, and pothole repairs, milling existing asphalt, and applying a new asphalt layer on specific street areas.

• Repair of subbase, base, and potholes.

• Milling of existing asphalt surface to a uniform depth of two (2) inches.

• Application of a new asphalt layer (S1 hot mix asphalt) with a compacted thickness of two (2) inches.

• Reinstallation of permanent pavement markings (lane lines, edge lines, crosswalks, symbols) using durable thermoplastic material.

• Adjustment of manhole covers, water valve boxes, and other utility castings to be flush with the new pavement surface.

• Site cleanup, removing all equipment, materials, and debris.

• Period of Performance: 30 calendar days (1 month).

• Place of Performance: Muñiz Air Base, Carolina, Puerto Rico 00979.

• Work Hours: Monday through Friday, 0730-1600, excluding recognized US holidays and government-directed closings.

• Quality Control (QC): Contractor must develop and maintain an effective QC Plan (QCP) and submit it within 10 days of award. A final QCP requires Contracting Officer (KO) acceptance.

• Quality Assurance (QA): Government will evaluate performance based on a Quality Assurance Surveillance Plan (QASP).

• Holidays: Contractor shall not perform services on recognized US holidays.

• Security: Compliance with all installation/facility access and local security policies. Background checks and PIV verification are required. Installation Entry Access List (EAL) processing takes 15-30 days. AT Level 1 Awareness Training is required within 10 days of contract start.

• Personnel Qualifications: Employees must possess required licenses for heavy equipment operation, welding, and Material Handling Equipment (MHE). The government does not provide training.

• Submittals: Includes Material Approval Submittals, Digging Permit, Environmental & Hazardous Material Plan, Quality Assurance Plan, Safety Plan, and Warranty Certificates. Specific due dates are provided.

• Site Visit: Contractors are responsible for conducting a site visit prior to proposal submission.

• Waste Management: All waste must be disposed of according to federal, state, and local regulations. Disposal slips are required.

• Personal Protective Equipment (PPE): Mandatory at all times while working on the project.

• Contractor Identification: Employees must identify themselves as contractors and wear identification badges.

• Combating Trafficking in Persons: Zero-tolerance policy applies. Training is required.

• Data Rights: Government owns all documents and materials produced under the contract.

• Organizational Conflicts of Interest (OCI): Contractors must notify the KO of any potential OCI and submit a mitigation plan.

• Health and Safety Plan (HSP): Site-specific HSP required, including certifications for asbestos and lead work. Must be submitted within 3 calendar days of any required change.
