BASIC MILITARY TRAINING METAL NAME TAGS

Statement of Work (SOW)

To procure finished metal U.S. Air Force (USAF) and U.S. Space Force (USSF) name tags for trainees and prior service personnel.

• Production of approximately 40,500 USAF/USSF name tags annually.

• Tags are for wear on the Air Force Service Dress Uniform (USAF) and Space Force Service Dress Uniform (USSF).

• USAF tags will display the trainee's last name.

• USSF tags will display the trainee's first/middle name on the first line and last name on the second line.

• Contractor is responsible for all personnel, equipment, tools, materials, and supervision.

• Orders will be placed via fax, email, or telephone.

• USAF Tags: Made of Brass or Nickel Silver (Name Plate) with Plastic (Attaching Insert).

  • Dimensions: 3/4 inch Height, 3 inches Width, 0.025 inch (Name Plate Thickness), 0.105 inch (Overall Thickness).
  • Finish: Nickel Plated, Silver Plated, or Matte with a brushed silver finish.
  • Attaching device: Prong and clutch type.
  • Lettering: Standard block or SL513 Portuguese font, 1/4 inch height, engraved, with epoxy-based paint matching Pantone Color 2747C.
  • Packaging: Protective plastic tube encased in a polybag (minimum 2 mil wall thickness).

• USSF Tags: Made of Red Brass 85/15 or Nickel Silver 65/18.

  • Dimensions: 3/4 inch Height, 3 inches Width, 0.025 inch (Name Plate Thickness), 0.105 inch (Overall Thickness).
  • Finish: Nickel plated (Red Brass only), silver plated, or matte with a brushed silver finish.
  • Attaching device: Prong and clutch type.
  • Lettering: Standard block or SL513 Portuguese font, 1/4 inch height, engraved, with epoxy-based paint matching Pantone Color 2188C.
  • Packaging: Protective plastic tube encased in a polybag (minimum 2 mil wall thickness).

• Both types of tags will have 15-degree angle cuts on the ends.

• Government will provide rosters weekly.

• AF/SF basic trainee orders delivery: within 14 calendar days of receipt.

• Prior service (EAOC) personnel orders delivery: within 3 business days of receipt.

• Re-orders/corrections delivery: within 3 business days of notification.

• Mistakes by the contractor will be corrected and delivered at no cost to the government.

• Errors made by the government will be corrected at government cost.

• Defective or misspelled name tags will be re-accomplished and delivered within three (3) business days of notification.

• Production Capacity: Seeks information on the capacity to produce, engrave, and deliver approximately 40,500 USAF tags and 930 USSF tags annually, with specific details on order frequency and size for each branch.
• Production & Delivery Lead Time: Asks for standard production and delivery lead times in calendar days.
• Small Business Status: Inquires about small business status as defined by the SBA, requesting certifications and primary NAICS codes if applicable.
• Interest in Solicitation: Asks if the company would be interested in submitting a quote/proposal in response to a future solicitation.
• SAM.gov Registration: Confirms active registration on SAM.gov, noting it's mandatory for receiving a Government contract, and requests CAGE Code and UEI if registered.

This questionnaire is crucial for potential bidders to:

• Determine if they meet the basic requirements for manufacturing these specific name tags.
• Understand the scale and nature of the requirement.
