Solicitation / RFP / RFQ

This document is a combined synopsis/solicitation (Request for Quotation - RFQ) for commercial items, specifically for the Indian Health Service (IHS) Digital Imaging System at the Supai Clinic. It serves as the sole solicitation, and written quotes are requested. Oral offers will not be accepted. All responding firms must be registered with the System for Award Management (SAM).

The requirement is for the following items:

• 17 each - UTGEU-33669-LTG STEEL UNICART 5DRW ALL LTG ELOCK (or equal)
• 34 each - DIV-DRW34 3" ADJUSTABLE CLEAR DIVIDER SYSTEM (or equal)
• 51 each - DIV-DRW5 DIVIDER SET, CRASH CART, 5" DEEP (or equal)

Items are to be shipped FOB Destination. Shipping costs must be included in the vendor quote and cannot be a separate line item. The government will not split the requirement.

• This procurement is for NEW Equipment ONLY; no remanufactured or "gray market" items.

• Vendor must be an Original Equipment Manufacturer (OEM) authorized dealer, distributor, or reseller.

• The quote MUST include a copy of the authorized distributor letter from the manufacturer.

• Delivery Location: Whiteriver Service Unit, 400 W. Apache Dr., Whiteriver, AZ 85941.

• Quotes must include all aspects of the RFQ (equipment, services, and options).

• Invoices must be prepared and submitted electronically via the Invoice Processing Platform (IPP).

• QUOTE DUE DATE: 03/20/2026 5:00 PM EST.

• The government intends to award a purchase order to the responsive and responsible vendor whose conforming response provides the lowest price technically acceptable.

• Evaluation procedures will be IAW FAR 13.106. The quote that represents the best benefit to the Government at a reasonable price will be selected.

• Technical solution will consist of: Price, Technical Acceptance of Product, and Delivery.

• Past performance will be evaluated using PPIRS and FAPIIS data.

• This requirement is a 100% Indian Small Business Economic Enterprise (ISBEE)/Native American small business set-aside on SAM.GOV.

• Offerors that do not fall into this category will not be considered for award.

• The document incorporates numerous FAR and HHSAR clauses by reference, covering various aspects such as representations and certifications, contract terms, payment, and small business requirements.

• This requirement is a 100% Indian Small Business Economic Enterprise (ISBEE)/Native American small business set-aside on SAM.GOV.

Sources Sought

This document is a Sources Sought notice issued by the Indian Health Service (IHS), Phoenix Area Office on behalf of the Whiteriver Service Unit. The purpose is to conduct market research to identify contractors capable of providing specific equipment. The IHS is determining if a small business set-aside is possible for this procurement or if full and open competitive procedures should be used.

The government is seeking quotes for the following NEW equipment ONLY:

• 17 each - UTGEU-33669-LTG STEEL UNICART 5DRW ALL LTG ELOCK (or equal)
• 34 each - DIV-DRW34 3" ADJUSTABLE CLEAR DIVIDER SYSTEM (or equal)
• 51 each - DIV-DRW5 DIVIDER SET, CRASH CART, 5" DEEP (or equal)

Vendors must be an Original Equipment Manufacturer (OEM) authorized dealer, distributor, or reseller. All equipment must be covered by the manufacturer's warranty, and warranty/service must be provided and maintained by the OEM.

• The government will not split this requirement.

• Quotes must include all aspects of the RFQ (equipment, services, options).

• Payment will be made through IPP (Invoicing Processing Platform).

• HHSAR 352.232-71 Electronic Submission of Invoice Payment Requests will apply.

• This is NOT a Request for Proposal (RFP).

• Responses should identify socio-economic status in accordance with SBA size standards.
