Statement of Work (SOW)

To provide services for the collection, packaging, and transportation of medical biohazard waste from Homestead Air Reserve Base, Florida, to a licensed treatment and disposal facility.

• Collection & Transportation: The contractor shall pick up medical biohazard waste monthly from the 482nd Medical Squadron, Building 347, Room 120, Homestead ARB, FL. Emergency pickups are also available.

• Packaging: The contractor must provide leak-resistant, moisture-impervious containers and bags (minimum three 3-mil red bags for rigid containers). At least two 30-gallon containers and appropriately sized bags are required.

• Disposal: Waste must be transported to a treatment facility licensed to accept medical and hazardous waste and treated to render it noninfectious and/or unrecognizable.

• Documentation: The contractor shall complete and provide a shipping manifest for each shipment, including details like manifest number, container count, facility information, transporter details, and certifications. The original manifest/certificate must be returned to the 482nd Medical Squadron upon completion of disposal to verify destruction.

• Monthly invoices are required.

• Copies of invoices, shipping manifests, and related documentation must be maintained and provided to the 482nd Medical Squadron within 72 hours upon request.

• Place of Performance: 482nd Medical Squadron, 12749 Elmendorf St., BLDG 347, Homestead ARB, FL 33039-1299.

• Hours of Operation: Pickups are scheduled monthly between 0730 and 1600, excluding federal holidays, at a mutually agreed-upon time.

• Personnel Security: Contractor personnel require base identification and vehicle passes. A written request detailing contract information and employee names must be submitted to the contracting officer. Personnel must comply with base regulations and AFI 31-101, Integrated Defense, and DODMAN 5200.02_AFMAN 16-1405 for unescorted access to controlled areas.

• Contractor Furnished Items: Containers, bags, labels, shipping manifest documents, state of Florida certifications for hauling biohazard waste, county business license for working within Homestead, FL, and valid liability insurance.

• Government Furnished Property: Building 347, Room 120, will be provided for storing contractor-furnished containers.

• Emergency Reporting: Contractor must provide an after-hours contact number/email for the Emergency Mass Notification System (EMNS) and comply with DAFMAN 10-206.

The 482nd Contracting Flight is conducting market research to identify potentially qualified contractors for Biohazard Waste Removal services at Homestead ARB, FL. Statement of Work (SOW) / Performance Work Statement (PWS)

To procure services for the pick-up and disposal of medical waste generated by the 482ND Medical Squadron at Homestead Air Reserve Base, Florida.

• Contractor shall provide all labor, tools, supplies, and equipment for medical waste removal and disposal.

• Services include collection, transport, treatment, and disposal of regulated medical waste.

• Scheduled pick-ups are once per month for Building 347 Biohazard Waste Storage.

• Emergency pick-ups are available as needed.

• All pick-up times to be coordinated with the end customer.

• Contractor must provide appropriate collection, storage, and shipping containers (preferred: 33-gallon capacity Medium Corrugated box).

• Containers must meet standards for occupational exposure to bloodborne pathogens and be labeled with appropriate regulatory markings.

• Waste must be packaged in accordance with 49 CFR Part 173.

• Contractor must provide a copy of the medical waste tracking form and certificate of destruction for each shipment.

• Containers must be marked with DOT Infectious Waste Substance label or OSHA biohazard marking.

• Waste to be transported via sealed truck to an EPA-approved disposal site.

• Waste must be destroyed in accordance with local, state, and federal regulations.

• Contractor must sign the manifest prior to departure and provide a copy to the 482nd Medical Squadron.

• Certificate of disposal must be provided within 45 days of waste pick-up, including specific shipment details.

• Contractor representatives must contact authorized personnel before and after service calls.

• Pick-ups are scheduled between 0730 and 1600 on pre-designated dates.

• Contractor is responsible for all applicable laws, regulations, and rules governing regulated medical waste.

• Contractor shall submit monthly invoices calculated at a firm fixed price per pick-up.

• Contractor is held professionally liable for errors and/or omissions.

• Compliance with Air Force safety standards, AFI 91-202, AFMAN91-203, AFI 91-207, and OSHA standards is required.

• Certificates of destruction must be provided within 45 days.

• Invoices must be received no later than five days after pick-up.

• The company must have been in the medical waste disposal business for a minimum of the past two years.

• Containers must be serialized, and serial numbers and pick-up dates must appear on tracking forms.

• Government will provide secure storage space for containers.
