• Wildfire Prevention & Suppression: Contractor is responsible for fire prevention and immediate suppression actions during contract performance.

• Industrial Fire Plan Execution: Contractor must designate an English-fluent Fire Guard responsible for executing the plan. This plan must be included in all subcontracts, and all contract employees must be informed of its conditions.

• Compliance Inspection: Forest Service will inspect for compliance. Deficiencies must be corrected within 24 hours, with activities halted until approved. Daily inspections by the Contractor's Fire Guard are required.

• Fire Prevention Requirements: Operations must adhere to Industrial Fire Plan levels, with specific restrictions on work hours, blasting, welding, incineration, refuse burning, and ash disposal. Controlled burns require a written Burn Plan and Forest Service approval.

• Fire Guard Duties: A trained Fire Guard is required for operations involving internal combustion engines, power-driven equipment, incineration, or camp/warming fires. The Fire Guard must be vigilant, able to take suppression action, and equipped with specific tools and communication devices.

• Wildfire Suppression: Contractor must take immediate action to suppress fires originating within the contract boundary and camp. Assistance may be provided to the Forest Service if trained and equipped. Independent suppression efforts outside the contract area require specific agreements.

• Equipment Requirements: All internal combustion engines must have approved spark arresters/mufflers. Vehicles and equipment must meet specific safety and maintenance standards. Powered hand tools require specific safety equipment and procedures.

• Smoking and Fires: Restrictions apply to smoking and open fires based on fire danger levels. Specific designated areas and permits may be required.

• Red Flag Events: Operations must adhere to specific Industrial Fire Precaution Plan levels (Stage D) during Red Flag Warnings or Alerts, potentially requiring shutdown.

• Violations: Non-compliance with fire plan requirements can result in immediate suspension of operations and potential criminal action.

• Fire Suppression Costs: The Forest Service reserves the right to participate in extinguishing fires caused by the Contractor's operations. Costs may be borne by the Contractor depending on the fire classification (Operations Fire vs. Negligent Fire).

• Adherence to daily inspections by the Fire Guard.

• Immediate correction of deficiencies within 24 hours.

• Compliance with specific fire precaution levels and restrictions based on daily Forest Service notification.

• Immediate suppression of fires originating within the contract area.

• Maintenance of required equipment in serviceable condition.

Statement of Work (SOW) / Performance Work Statement (PWS)

To remodel specific buildings (Mod A #11-33, Mod B #11-60, and Range house #11-38) located on the Black Mesa Ranger District.

This SOW details the remodeling of bathrooms, kitchens, and general interior spaces across three buildings. Key tasks include:

• Flooring: Replacement of carpet with high-traffic carpet tiles (approx. 625 sq ft) and installation of waterproof vinyl flooring in bathrooms (approx. 398 sq ft) in buildings 11-33 and 11-60. New flooring (rigid core vinyl plank and tile) in building 11-38. New flooring in restrooms of building 11-38.

• Bathrooms (11-33): Sealing windows, drywall repair, installation of new toilets, supply lines, exhaust fans, lights, doors, and wainscot wall treatment.

• Bathrooms (11-60): Removal and disposal of existing fixtures, installation of new flooring, fixtures, sink, medicine cabinet, toilet, supply lines, exhaust fan, light, wainscot wall treatment, and painting.

• Doors (11-33 & 11-60): Replacement of interior doors (excluding closet doors for furnace/water heater and pantry) with new hardware.

• Restrooms (11-38): Installation of new tile shower surround, vanity, fixtures, sink, mirror, grab bars, lighting, texture, paint, window trim, outlet covers, and flooring with a smooth transition. Includes installation of 2 new sliding closet doors.

• Kitchens (11-38): Replacement of cabinets (approx. 23 linear ft), countertops (approx. 41 sq ft) with granite or natural stone, sink, faucet, backsplash, lighting, paint, vent hood, and breakfast bar seating extension. Includes replacement of 7 interior doors with solid core doors.

• Water Heaters: Installation of new propane gas water heaters and supply lines in buildings 11-38, 11-33, and 11-60, including disposal of old units.

• General: Wall painting, baseboard and trim installation/replacement, ceiling tile replacement, blind replacement (18 windows), and final cleaning.

• Location: Black Mesa Administrative Site, 2748 E. HWY 260 Overgaard, AZ 85933.

• Work Schedule: Monday through Friday, 0700 – 1700. Changes require Contracting Officer approval.

• All work must be in accordance with contract drawings and specifications.

• Site visits are strongly recommended prior to bidding.

• Field verification of site-specific utilities (plumbing, electrical) is required.

• Substituted items or "approved equals" require written approval from the Contracting Officer Representative (COR) prior to purchase.

• Contractor must submit a schedule and proposed work hours within 5 calendar days of a written request.
