• Foreign vendors are not required to be registered in SAM but must complete AMS-Form-050 (Enclosure B).

• Enclosures include: Draft Purchase Order 480029, AMS Form 050 Reps and Certs, AMS Form 043 Quotation Pricing Sheet, Vendor ACH Authorization Form, and W-9 Form.

Statement of Work (SOW) / Performance Work Statement (PWS)

To provide monthly secure shredding services for paper-based materials on-site at Brookhaven National Laboratory (BNL).

• Provide monthly secure shredding services for paper-based materials.

• Utilize Contractor-provided containers/bins (68 or 95 gallon, or equivalent) placed on-site at BNL.

• Service containers/bins monthly; bin amounts are adjustable by the BNL Technical Representative.

• Contractor must be AAA Certified by NAID.

• Services must comply with DOE Order 471.7 or successors for Controlled Unclassified Information (CUI) destruction.

• Destruction methods include:

  • Single-step paper destruction using cross-cut shredders producing 1 mm x 5 mm particles (or smaller), OR disintegrator devices with a 3/32 in. (2.4 mm) security screen.
  • Multi-step paper destruction where the resulting CUI is unreadable, indecipherable, and irrecoverable. Recycling is permissible only if CUI is transformed into new paper products (e.g., paper towels).

• BNL may conduct periodic surveillance to verify CUI compliance.

• An invoice and certificate of destruction will be issued monthly.

• Bins will be inspected for damage and replaced as necessary.

• Badging and Training: All personnel require a BNL non-construction contractor badge (annual renewal) and must attend BNL Contractor Vendor Orientation (CVO) training (annual renewal). CVO training is offered virtually on Mondays, Wednesdays, and Fridays at 8:30 am.

• Access Requirements: Personnel entering posted Controlled Areas must meet specific requirements, including up-to-date General Employee Radiation Training (GERT) or Radiation Worker (RW-1) and facility-specific training for unescorted access. Escorted access requires pre-approval and coordination with Ann-Marie Luhrs.

• Personal Protective Equipment (PPE): Required PPE includes safety work boots (steel toe), Hi-Vis safety vest, safety glasses, and work gloves.
