This document is a list of biomedical equipment inventory for USNMRTC Yokosuka, USNMRTU Iwakuni, and USNMRTD Diego Garcia. It details the equipment's ECN, Manufacturer Serial Number, Nameplate Model, Nomenclature, and Manufacturer.

• Equipment identification numbers (ECN, Serial Number, Model)
• Equipment name (Nomenclature)
• Manufacturer
• Lists equipment such as flowmeters, testers (ventilator, electrical safety, defibrillator, pulse oximetry), simulators (multiparameter, noninvasive blood pressure), multimeters, and radiographic quality assurance devices.

This inventory list may be relevant to bidders if the opportunity involves the maintenance, calibration, repair, or replacement of biomedical equipment at these specific locations. It provides insight into the types and quantities of equipment currently in use, which could inform proposals related to asset management, service contracts, or equipment procurement.

Statement of Work (SOW) / Performance Work Statement (PWS)

To provide off-site calibration and preventive maintenance services for biomedical equipment used by the United States Naval Hospital Yokosuka (USNHY) and its Branch Health Clinics (BHC) in Iwakuni and Diego Garcia. This effort is required because annual off-site calibration and maintenance are not available from internal Biomed resources.

• Provide trained technicians to perform calibration and preventive maintenance on listed equipment.

• Furnish all supplies and transportation for technicians.

• Maintain equipment to original equipment manufacturer (OEM) specifications, adhering to Federal, State, and Local laws, U.S. Navy regulations, DHA instructions, and Joint Commission requirements.

• Calibrate equipment to ISO 17025 standards and ANSI Z540.3.

• Use OEM-trained or NASED-certified personnel for repairs, requiring a quote and government approval before proceeding.

• Deliverables and tasks must be completed within the scope of the PWS.

• No incorrect or inadequate assumptions in review and analysis.

• No oversights in report development that cause delays.

• All written documentation must be in a skilled business style and conform to established standards.

• Work must be completed within agreed-upon timeframes.

• Quality of work and content of deliverables must be highly professional.

• Period of Performance: 12-month base year, including four option periods and a six (6) month extension.

• Place of Performance: Factory at the contract awarded company and/or subsidiary company, or at a subcontractor's factory.

• Field Service Reports: Submit an itemized list of corrective maintenance work and calibration certification upon service completion.

• Equipment Replacement: The Government reserves the right to replace equipment on a one-for-one basis if the replacement performs a similar function and does not significantly change the scope of services.

• Documentation and Performance: USNHY has unlimited rights to all deliverables. The Government has five (5) business days to review and provide comments on deliverables, and the contractor has five (5) business days to incorporate them.

• Contractor Point of Contact (POC): A primary POC must be designated, readily available, and responsive to Government inquiries. Changes to the POC must be communicated in writing within two (2) business days.

• Invoice Instruction: Submit invoices with Contract Number/Delivery Order Number at PIEE/WAWF no later than 30 days after work is performed.
