This document provides responses to vendor questions regarding the solicitation HT940626QE014 for Continued Readiness Program and Life Safety Assessment services.

• Contract Status: Services were previously performed under an expired contract (N6890821P0034), but there is no current contract.

• Building 2034: The square footage is 19,143 sq ft. A physical walk-through is likely necessary for familiarity and to capture updates in life safety drawings. PWS 5.1.2(e) has been updated to include this building.

• Statement of Conditions Touring: Comprehensive touring for the Statement of Conditions will be conducted simultaneously with the quarterly onsite Continued Readiness Program visits. PWS 5.1.3.5 has been added for clarity.

• Quote Submission: Quotes should be emailed to ryan.t.poe.civ@health.mil and andrea.l.giuliano.civ@health.mil, referencing the solicitation number. Volume IV must remain in Excel format.

• Quote Submission Deadline: Offers are due April 2, 2026, at 10:00 AM ET.

• Question Submission Deadline: Questions were due March 19, 2026, at 10:00 AM ET.

This amendment provides answers to questions, updates ADDENDUM TO 52.212-1 (Instructions to Offerors), and revises sections 5.1.2(e) and 5.1.3.5 of the Performance Work Statement (PWS).

Performance Work Statement (PWS)

To provide Continued Readiness Program (CRP) and Life Safety Assessment services for the Environment of Care (EOC) Program at Naval Hospital Jacksonville (NHJAX), Florida. The goal is to ensure NHJAX maintains safety management systems in accordance with Joint Commission and NFPA 101 Life Safety Code standards, promoting a culture of safety and preparedness for audits.

• Conduct comprehensive audits and on-site inspections of EOC components: Safety, Security, Fire Safety, Hazardous Materials and Waste, Medical Equipment, Utilities Management, and Emergency Management.

• Review processes and report deficiencies, identifying strengths and weaknesses, and educating staff.

• Perform four (4) scheduled on-site visits annually, each lasting four (4) days, totaling sixteen (16) on-site days per year.

• Provide updated Life Safety drawings and a comprehensive Statement of Conditions assessment to determine compliance with NFPA 101 Life Safety Code 2024.

• Develop a Plan for Improvement (PFI) based on assessment findings.

• Provide telephone support during Joint Commission inspections.

• Four (4) site assessments per year, 4 days each.

• Written reports submitted within 14 days of each site assessment.

• Updated Life Safety Drawings provided twice per year.

• Comprehensive Statement of Conditions assessment completed during each performance period (except when TJC visits).

• Contractor must have experience with NFPA 101 Life Safety Code 2024 and The Joint Commission (TJC) EOC standards; assessment team members require a minimum of four (4) years of experience with TJC standards.

• Base period of twelve (12) months with four (4) 12-month option periods, and a potential six (6) month extension.

• Place of Performance: Naval Hospital Jacksonville, Building 2080 Child St., Jacksonville, FL 32214.

• Work hours: Monday through Friday, 0700 to 1600 hours, excluding Federal holidays.

• Contractor must develop and maintain a Quality Control (QC) program.

• Contractor personnel require base access coordination and must adhere to base regulations and security protocols.

• Compliance with DoD cybersecurity and personnel security requirements (HIPAA, PII/PHI protection) is mandatory.

• Contractor must identify themselves as contractor support personnel in all communications.

• Life Safety CAD drawings, access to spaces, keys, security badges.

• Office space and loaned office supplies/computers during on-site visits.

• Quality Control Program: Within 30 days of award.

• Comprehensive Statement of Conditions Assessment Document: Within 2 weeks of assessment.

• Site Visit Assessment Report: Within 14 days of assessment completion.

• Draft and Final Updated Life Safety Drawings: Twice per year.

• Plan for Improvement: Within 14 days of submitting Statement of Conditions Compliance document.

Solicitation / RFP / RFQ

This solicitation is a Request for Quote (RFQ) for "Continued Readiness Program and Life Safety Assessment services" for the Facilities Management Department at Naval Hospital Jacksonville, Florida. The acquisition is set aside for Small Business.

  • Past Performance: Evaluated on an Acceptable/Unacceptable/Neutral basis, considering recent and relevant contracts.
  • Price: Evaluated for reasonableness and compatibility with technical requirements.

Performance Work Statement (PWS)

To provide Continued Readiness Program (CRP) and Life Safety Assessment services for the Environment of Care (EOC) Program at Naval Hospital Jacksonville (NHJAX) to ensure compliance with Joint Commission and NFPA 101 Life Safety Code standards.

• Conduct four (4) scheduled on-site visits annually, each lasting four (4) days, for a total of sixteen (16) on-site days per year.

• Perform comprehensive audits and inspections of EOC components: Safety, Security, Fire Safety, Hazardous Materials and Waste, Medical Equipment, Utilities Management, and Emergency Management.

• Verify compliance of records and documents, including Management Plans, Risk Assessments, audits, annual evaluations, and committee activities.

• Conduct physical site evaluations, hazard surveillance tours, and random staff challenges.

• Perform life safety walk-throughs to assess physical readiness (e.g., smoke barriers, rated walls).

• Develop and update Life Safety Drawings twice annually for Building 2080 (hospital) and Building 2034 (substance abuse rehab facility).

• Complete a Comprehensive Statement of Conditions assessment annually (unless The Joint Commission visits) to certify compliance with NFPA 101 standards.

• Develop a Plan for Improvement (PFI) detailing deficiencies, proposed actions, timelines, and estimated costs.

• Provide telephone support during Joint Commission inspections.

• Written reports submitted within 14 calendar days after each site visit.

• Updated Life Safety Drawings provided twice per year.

• Comprehensive Statement of Conditions document completed annually.

• Plan for Improvement documentation submitted electronically.

• Contractor's assessment team must have a majority with at least four (4) years of experience with TJC standards and the Life Safety Code.

• Base period of twelve (12) months with four (4) 12-month option periods, plus a potential six (6) month extension.
