Performance Work Statement (PWS)

To establish requirements for providing pre-employment physical examinations for new hires of the Defense Logistics Agency (DLA) at Robins Air Force Base, Georgia, ensuring selected candidates meet necessary medical and physical standards.

The contractor shall provide professional medical services, facilities, equipment, and personnel to perform comprehensive physical examinations. Services include, but are not limited to:

• Routine physical examinations

• Audiograms (hearing tests)

• Vision screening

• Blood pressure and vital signs

• Respiratory assessments (if applicable)

• Drug testing (urinalysis)

• Immunizations (as required)

• Review of medical history

• Completion of required documentation

• Physicals must be performed within 5 business days of request.

• Results and documentation must be provided within 3 business days of exam completion.

• Contractor must ensure privacy and confidentiality of all medical records.

• Contractor must be certified and/or licensed in Georgia and comply with DOD and DLA medical standards.

• Performance will be evaluated based on timeliness, completeness, accuracy, professionalism, and compliance with privacy laws and medical standards.

• Period of Performance: See award.

• Place of Performance: Contractor's medical facility within reasonable distance to Robins Air Force Base. Facility must be licensed and comply with OSHA, HIPAA, and Georgia health codes.

• Contractor must provide all medical supplies, equipment, qualified/licensed personnel, and compliant facilities.

• Contractor must ensure secure transmission of medical documentation.

• Contractor personnel must comply with base access protocols for Robins Air Force Base.

Solicitation / RFP / RFQ

This is a combined synopsis/solicitation for commercial services, specifically pre-employment medical physicals for DLA Distribution Warner Robins, GA (DDWG). It is issued as a Request for Quote (RFQ) under solicitation number SP3300-26-Q-0092, prepared in accordance with FAR Part 12.
