This is a Request for Quotation (RFQ) for a device inspection and service plan. The government anticipates awarding a firm-fixed-price, stand-alone purchase order.

• The primary requirement is for a device inspection and service plan, as detailed in Attachment 1 (Performance Work Statement and QASP).

• A "Device Service Plan" is also mentioned as an option CLIN.

• Pre-inspection is required within 45 days of the period of performance start date.

• The anticipated contract type is firm-fixed-price.

• The period of performance is anticipated to be a base period of 2 months (06/01/2026 to 07/31/2026) and an option period of 10 months (08/01/2026 to 05/31/2027).

• The total duration of the contract shall not exceed one year.

• Quotes must be submitted by 05/15/2026 at 0900 ET.

• Quotes must hold prices firm for 30 calendar days from the receipt date.

• The evaluation will be based on the "Lowest Price Technically Acceptable" (LPTA) criteria.

• Technical Acceptability will be rated as "acceptable" or "unacceptable" based on the offeror's ability to demonstrate a sound and compliant approach.

• Past Performance will be evaluated as "acceptable" or "neutral."

• Pricing must be provided for all line items and will be evaluated for fairness and reasonableness.

• The SF1449 RFQ document must be completed, including specific blocks and a yellow checkbox declaration of technical capability and acceptance of terms.

• Inquiries must be submitted via email up to 48 hours before the due date.

• This acquisition is listed as "UNRESTRICTED."

• All solicitation documents and amendments will be posted on sam.gov.

• The government reserves the right to cancel the solicitation.

• Historical contract information will not be provided.

Performance Work Statement (PWS)

To provide a pre-inspection and an optional device service plan for one BD FACSMelody and FACStation with two lasers.

• Perform a pre-inspection of the specified BD FACSMelody and FACStation.

• Provide a Device Service Plan, including on-site preventative maintenance, software revisions, unlimited business day service visits, 48-hour guaranteed emergency response, and unlimited telephone support.

• The work is performance-based and firm-fixed-price.

• The contractor must perform to the requirements and standards established in the PWS.

• A Quality Assurance Surveillance Plan (QASP) will be used to evaluate performance, with an Acceptable Quality Level (AQL) of 100% for technical requirements and tasks.

• Service is performed on-site at a USDA Facility, 600 EAST MERMAID LANE, WYNDMOOR PA 19038.

• CLIN 0001 (pre-inspection) is for 45 days.

• CLIN 0002 (option for Device Service Plan) is for 10 months.

• Contractor personnel must be OEM authorized repair technicians.

• All contractor personnel require USDA escort while on location.

• Invoicing and payment are processed through the Department of Treasury's Invoice Processing Platform (IPP).

• The firm fixed price includes one Preventative Maintenance Kit per CLIN.

• Consumable items are not included in the firm fixed price.

• Out-of-scope repairs are not included and must be addressed separately.
