Statement of Work (SOW)

To procure Electromyography (EMG) and Nerve Conduction Velocity (NCV) testing services for Adult In Custody (AIC) individuals at the Federal Medical Center (FMC) Lexington.

• Provide EMG/NCV testing services for upper and/or lower extremities, including cases with spinal involvement and suspected multi-limb polyneuropathies.

• Services must conform to community standards and all applicable laws and regulations.

• Specific study types include one, two, three, or four extremity EMG/NCV studies.

• The contractor will use EMG/NCV machines provided by FMC Lexington and will be supplied with expendable materials.

• Detailed results and recommendations must be provided in a written report after each study.

• Services must be performed on short notice, with a response time of within four hours when required.

• The contractor is responsible for completing and maintaining the Session-Based Invoice Verification (SBIV) for each visit.

• Services to be performed within the confines of FMC Lexington's Rehabilitation and Orthopaedic Department and/or administrative building.

• Work schedule will be between 7:30 am and 3:00 pm, Monday through Friday, excluding Federal Holidays, subject to mutual agreement.

• FMC Lexington anticipates approximately thirty (30) sessions per year, but reserves the right to adjust this number.

• Education and Experience: Provider must be a licensed physician or licensed physical therapist with a current unrestricted professional license in Kentucky. Must have at least three years of documented experience and provide documentation of qualifications (diplomas, certifications, resume, etc.).

• Work Attire: Professional attire required; khaki or green clothing is prohibited.

• AIC Management: Contractor is not responsible for inmate management, accountability, or supplies. Must maintain awareness of security and report infractions.

• Communication: Must not discuss findings or treatment with AICs; all recommendations go in the written report. No treatment outside the scope of consultation without prior authorization.

• Health Requirements: Contractor must be free of communicable diseases and physically able. Must provide proof of Tuberculosis testing and Hepatitis B vaccination (or signed declination).

• Institutional Security: Must adhere to all institution regulations, attend initial and annual security training, and wear an issued badge.

• Reporting: Must provide a monthly invoice/statement of service rendered, including SBIV details.

• Notice of Dissolution: A six-month notice is preferred if the contractor chooses to dissolve the contract.
