Performance & Provider Requirements

  • Scheduling & Volume: Clinics scheduled by mutual agreement; approximately thirty (30) sessions per year anticipated, with flexibility. Work schedule 7:30 am - 3:00 pm, M-F (excluding Federal Holidays). Short-notice response (within four hours) may be required.
  • Provider Qualifications: Must be a licensed physician or physical therapist in Kentucky with a current unrestricted license and at least three years of documented EMG/NCV experience.
  • Institutional Requirements: Adherence to security regulations, security training, badge wearing, professional attire (no khaki/green). Contractor is not responsible for inmate management. Health requirements include TB testing and Hepatitis B vaccination proof/declination.
  • Reporting: Monthly invoices and Session-Based Invoice Verification (SBIV) for each visit.
