Quality Assurance Surveillance Plan (QASP)

This document outlines the Quality Assurance Surveillance Plan (QASP) for the Water Monitoring Service contract at the Salem Veterans Affairs Medical Center (VAMC), Contract Number 36C24626P0617. It details the systematic method the Government will use to evaluate the contractor's performance.

• Monitoring Scope: Defines what aspects of the contractor's performance will be monitored.

• Monitoring Methods: Explains how the monitoring will take place (e.g., direct observation, complaint review, user surveys).

• Monitoring Personnel: Identifies who will conduct the surveillance (Contracting Officer - CO, Point of Contact - POC).

The QASP establishes performance standards and Acceptable Quality Levels (AQLs) to assess contractor performance against the Performance Work Statement (PWS). Key performance areas include:

• Adherence to PWS requirements (Section B.2).
• Successful provision of required services.
• Conformance to inspector qualifications and performance standards.
• Compliance with reporting requirements.

AQLs: Generally set at 95% for service provision and conformance, and 100% for reporting requirements.

Surveillance Methods: CO/POC periodic surveillance, observation, and audit.

• Incentives: Favorable contractor performance evaluations.

• Disincentives: Unfavorable evaluations, Contract Discrepancy Reports (CDRs) for failures, potential corrective actions, or termination procedures for consistent issues.

• Acceptable Performance: Will be documented.

• Unacceptable Performance: Will be communicated to the CO, potentially via a Contract Discrepancy Report (CDR), requiring contractor acknowledgment and possible corrective action plans.

• The POC will meet with the contractor monthly to assess performance and provide a written assessment.

Solicitation / RFP / RFQ

This solicitation is for Groundwater Monitoring Services at the Salem VA Medical Center Landfill. The objective is to provide onsite environmental services to conduct landfill groundwater monitoring in compliance with Virginia Solid Waste Management Regulations.

• Conduct semiannual groundwater testing of 5 wells.

• Complete and submit semiannual groundwater monitoring reports to the Virginia Department of Environmental Quality (DEQ).

• Include all laboratory testing fees and costs in the proposed price.

• Services are to be performed during normal business hours (Monday-Friday, 8:00 AM - 4:30 PM, excluding federal holidays).

• Contract Type: Firm-Fixed-Price.

• Period of Performance: Base year (06-15-2026 to 06-14-2027) with four (4) one-year option periods.

• Estimated Value: $19 Million (for the base period and options).

• NAICS Code: 541330 (Engineering Services).

• Quotes are due by 10:00 AM EST on June 5, 2026, submitted via email to Kristine Woodbury (kristine.woodbury@va.gov).

• Quotes must be assembled into specific sections: Quote Form (SF 1449), Pricing, Representations and Certifications, Technical/Management Approach, and Past Performance.

• Technical Volume is limited to 25 pages.

• Technical questions are due by 10:00 AM EST on June 1, 2026.

• Award will be based on the best value, considering Technical/Management Approach, Past Performance, and Price.

• This acquisition is a total small business set-aside.

• Offerors MUST be a verified Service-Disabled Veteran Owned Small Business (SDVOSB) at the time of quote submittal.

• Offerors must be registered and current in the System for Award Management (SAM).

• Offerors must submit a completed and signed copy of VA NOTICE OF LIMITATIONS ON SUBCONTRACTING—CERTIFICATE OF COMPLIANCE FOR SERVICES AND CONSTRUCTION (VAAR 852.219-75).
