This Performance Work Statement (PWS) outlines the requirements for providing non-personal services for forklift inspection, preventative maintenance, and repair services for Joint Base San Antonio (JBSA) Lackland and Port San Antonio.

• Provide management, tools, maintenance manuals, supplies, equipment, parts, and labor for forklift services.

• Perform quarterly inspections/Preventative Maintenance (PM) at 90, 180, 270, and 360-day intervals, or as specified by maintenance manuals.

• Provide emergency repair services within 48 hours of notification for equipment that becomes inoperable or unsafe.

• Replacement parts identified during inspections or emergency repairs will be billed against CLIN X016.

• Submit inspection reports quarterly, detailing performed inspections, scheduled inspections, and a history of emergency repairs and parts replaced.

• Emergency repair response time: within 48 hours of notification (excluding weekends/holidays).

• Work must be performed in accordance with commercial and industrial standards and specified maintenance manuals.

• Services are for forklifts located at Port San Antonio and JBSA Lackland.

• Work is to be performed during normal duty hours (7:30 AM to 4:30 PM, Monday-Friday, excluding Federal Holidays), unless pre-approval for outside hours is obtained.

• Contractor must furnish all necessary materials, equipment, and services.

• Contractor personnel must be U.S. citizens and subject to National Crime Information Checks (NCIC).

• Contractor is responsible for waste removal and disposal, including hazardous waste manifests.

• Contractor must register with the BASE HAZMAT PHARMACY before bringing chemicals onto the base.

• Access to JBSA Lackland is through the Growden Rd gate, with potential access time up to 45 minutes.

• Appendices A-E detail the Forklift Equipment List and specific maintenance requirements for 90/250, 180/500, 270/750, and 360/1000 day/hour intervals.

This document is a combined synopsis/solicitation (RFQ) for Forklift Inspection, Preventative Maintenance, and Repair Services. The services are for the Air Force Life Cycle Management Center (AFLCMC) Logistics Branch (HNCL) at Port San Antonio and Joint Base San Antonio (JBSA) Lackland. The contractor will provide non-personal services including management, tools, maintenance manuals, supplies, equipment, parts, and labor. The acquisition is a 100% Total Set-Aside for small business concerns.
