• General Comment: An overall assessment of the contractor's ability to execute their proposal.

This questionnaire is sent to entities that have previously contracted with potential offerors. Bidders should be aware that their past performance on similar contracts will be evaluated. The feedback provided by respondents directly influences the source selection decision. The deadline for submitting responses is 10 calendar days after receipt of the letter.

Amendment / Modification

This document serves as an addendum to RFO 52.212-1 and RFO 52.212-2, providing additions and changes to FAR provision 52.212-1 and RFO 52.212-2.

Key changes and additions include:

• Requests for Information (RFIs): Clarifies procedures for submitting RFIs, including deadlines for initial solicitation RFIs and RFIs to subsequent amendments.

• Content Requirements for Quotes: Details the necessary information for the Cover Page, Technical Approach, Past Performance Information, and Price Matrix sections.
• Technical Approach: Elaborates on the specific areas the technical approach must address, including processes for flooring replacement, waste disposal, base pass requests, moisture content verification, understanding of the Carpet II Program, management approach, and organizational structure.
• Past Performance Information: Specifies the requirement for 3-5 references and the information to be provided for each. It also clarifies the process for submitting Past Performance Questionnaires.
• Price Matrix: Outlines the structure of the IDIQ Price Matrix, including pre-established prices and rates, and pricing for sample task orders. It emphasizes that all prices must be in U.S. dollars and details requirements for percentage rates. It also warns that failure to meet formatting criteria or propose prices for all elements may result in the quote being considered unacceptable.
• Assumptions: Requires firms to clearly state any pricing-related assumptions, conditions, or exceptions in a designated section, otherwise, the quote will be assumed to have none.
• Guarantee of IDIQ rates and Contract Issuance: States that proposed IDIQ rates must be guaranteed through December 31, 2026.
• Sample Solicitation Task Orders: Informs bidders that sample task orders are included to demonstrate the application of prices, and at least one may be awarded.
• Evaluation Basis (RFO 52.212-2 Addendum): Clarifies that the Government intends to make one award, with a preference for an award encompassing services at both MacDill AFB and Avon Park Air Force Range (APAFR). It details the Best Value evaluation basis, prioritizing Technical Capability, Past Performance, and Price. Bidders must ensure their quotes strictly adhere to the updated document submission requirements, formatting guidelines, and content specifications. Failure to comply may result in the quote being deemed non-responsive or ineligible for award. The evaluation criteria and basis of award have also been clarified.

Solicitation / RFP / RFQ

This is a combined synopsis/solicitation (Request for Quote - RFQ) for flooring replacement and installation services for commercial products or services. The acquisition is set-aside for small business concerns. The government intends to award a single Indefinite Delivery, Indefinite Quantity (IDIQ) contract with a ceiling value of $9,000,000.

• The scope includes all services and materials necessary for flooring installation, which may include replacement of existing flooring or initial installation.

• Materials to be provided include, but are not limited to, carpet, VCT, LVP, LVT, rubber flooring, adhesives, rubber cove base, ceramic/porcelain/quarry tile, grout, seam sealers, floor patch, transition strips, and wood base sealers.

• The contractor must provide all equipment, tools, labor, transportation, and off-base storage.

• For carpet installation, materials must be sourced from Air Force Carpet Program manufacturers unless otherwise specified.

• The contractor must demonstrate understanding of processes for flooring replacement, waste disposal, base pass requesting, and verifying concrete floor moisture content.

• Contract Type: IDIQ

• Period of Performance: Base year plus six option years, anticipated to begin approximately July 1, 2026. Includes a six-month extension of services under RFO 52.217-8.

• Ceiling Value: $9,000,000

• Requests for Information (RFIs) are due by 12:00 PM EDT on May 11, 2026.

• Offers are due by 12:00 PM Eastern Daylight Time on June 4, 2026.

Performance Work Statement (PWS)

This PWS outlines the requirements for flooring installation and removal services at MacDill AFB and Avon Park Air Force Range (APAFR).

• Services include installation, removal, and refinishing of various flooring types (carpet, VCT, LVP, LVT, rubber, ceramic/porcelain tile, wood base).

• Materials to be provided by the contractor, including flooring, adhesives, cove base, fasteners, etc.

• Contractor is responsible for all equipment, tools, labor, transportation, and off-base storage.

• Work includes moving and relocating standard office furniture and systems furniture.

• Removal of existing flooring, installation of new flooring, sanding/finishing of wood base, and removal/installation of stair treads and risers.

• Thorough cleaning of installation areas, including debris removal and surface preparation.

• Disposal of all waste materials and debris at a state-permitted site.

• Work will occur in occupied buildings, requiring coordination with occupants.

• Performance Objectives (PO) include removal of existing flooring, installation of new flooring, cleanup/disposal, furnishing materials, vacuuming, environmental requirements, and quality control.

• Performance thresholds are defined with "Excellent" (0 defects) to "Unsatisfactory" (more than 5 defects) ratings for most objectives.

• Surveillance methods include customer complaint and 100% inspection/periodic validation.

• Locations: MacDill AFB, Florida and Avon Park Air Force Range, Florida.

• Contractor must submit a Hazardous Material (HM) Handling Plan.

• No asbestos-containing materials are to be used.

• Contractor must comply with environmental regulations regarding hazardous waste and recycling.

• Government will furnish utilities; contractor must conserve use.

• Contractor must comply with all security requirements, including background checks for personnel working on MacDill AFB.

• Site Foreman/Superintendent must have 5 years of experience, with 2 years as foreman/superintendent, and hazard identification/safety compliance experience.

• Site Foreman/Superintendent and Quality Control Personnel must have Certified Floorcovering Installers (CFI) certification.

• Removal of existing glue-down carpet, wall base, stair nosing, and transitions.

• Any additional preparation due to unforeseen conditions must be brought to the attention of the Contracting Officer Representative (COR).

• If Asbestos Containing Material (ACM) is encountered, work disturbing ACM must cease, and the COR must be immediately notified.

• Protect adjacent property, building, and contents from dust, dirt, or other materials.

• Daily cleanup of trash, debris, refuse, and garbage, placing it in appropriate containers.

• Building 565, 1st Floor.

• Work to commence within 30 days and be completed within 60 days of purchase order award.

• Contractor must immediately notify COR of delays due to mandated carpet goods suppliers.

• Demolish existing transitions and wall base.

• Inspect existing VCT for loose tiles; remove and fill gaps with approved patching compound if necessary.

• Prepare the VCT subfloor by stripping wax and floor finish, and ensuring it is free of dirt, oil, and grease. Leveling compound may be used if height differences or grout lines are significant.

• Protect adjacent property from dust and debris.

• Daily cleanup of trash, debris, and refuse.

• Work must be completed within 1 month from the start date.

• All building material from demolition must be disposed of off Avon Park AFR.

• If Asbestos Containing Material (ACM) is encountered, work must cease immediately, and the COR notified.

• Location: Building 26, Avon Park AFR.

• Start: Within 30 days of purchase order award.

• Completion: Within 60 days of purchase order award.

• "Pressure-sensitive" adhesive to be used.

• Notify COR immediately of any delays due to mandated carpet goods suppliers.

• The VCT floor must be free of wax before adhesive application.
