- IEE Representation Form: ISBEEs and IEEs must also submit a completed IHS IEE Representation Form (attached to the original notice) in accordance with HHSAR 326.606-1(b). This form requires self-certification of Indian Economic Enterprise status, including tribal entity name, 51% owner, business name, DUNS, and certifying signature. Offerors must meet the IEE definition throughout the offer, award, and performance periods.
