Scope of Work
The contractor will provide comprehensive Medical Respite Care Services, including:
- Secure and sanitary respite housing: ADA-accessible dorm or single rooms (max double occupancy), available 24/7, within 15 miles of the VA Portland Medical Center.
- Basic necessities: Hygiene supplies, clothing, and food.
- Integrated support: Access to onsite Primary Care, referrals, housing plan development, and assistance with transitional or permanent supportive housing.
- Case management: For complex needs across health, housing, employment, and benefits, including medical case management, medication management, employment services, and financial planning.
- Therapeutic and rehabilitative services: Veteran-centered case management, recovery support, and discharge planning focused on housing stability.
- Appointment support: Assistance with making and keeping appointments, including transportation.
- Specialized care access: Support for substance use disorder treatment, Hepatitis C treatment, and specialized mental health care.
- Exclusions: The contractor will not be required to provide detoxification or other acute level treatment. Veterans served must be medically independent and able to perform all activities of daily living without personal assistance from shelter staff.
Performance Standards
