Statement of Work (SOW)

To provide transitional housing and supportive services for homeless veterans requiring a transition program, accommodating ten (10) beds for room and board.

• Provide room and board for 10 veterans, including furnishings and linens.

• Provide two balanced meals daily and nutritious snacks.

• Offer laundry facilities for residents.

• Provide personal hygiene items.

• Offer supportive services including structured group activities, health and hygiene instruction, medication storage, support for an alcohol/drug-free lifestyle, and assistance with transportation to appointments.

• Services are to be provided 7 days a week, including Federal Holidays.

• Availability of 10 beds for homeless veterans (100%)

• Unplanned discharges back to a homeless situation (95%)

• Injury to veterans while in contractor's care (100% for minor injuries reported monthly, immediate reporting for major injuries)

• Resolution of veteran complaints (90% or greater)

• Timeliness in response to information requests (within 3 days)

• Period of Performance: One year base period with four (4) one-year options.

• Place of Performance: Within 10 miles of the Boise VA Medical Center.

• Facility must be equipped with operational air conditioning/heating, be clean, and have functioning safety equipment (first aid kit, fire extinguisher, alarms).

• Contractor must comply with VA policies, including safety, patient care, infection control, and information security.

• Contractor personnel must not have access to VA sensitive or computer information and will not require routine access to VA Facilities; intermittent access will be escorted.

• Contractor must review the OIG List of Excluded Individuals/Entities and certify compliance with HIPAA.

• Contractor must provide documentation of Tuberculosis Testing (PPD) for all personnel.

• Contractor must comply with OSHA regulations regarding occupational exposure to blood-borne pathogens.

• Contractor must provide a Certificate of Insurance for bodily injury and property damage liability.
