Statement of Work (SOW) / Performance Work Statement (PWS)

To outline the requirements for on-site hospital-grade mattress inspection services at the Saint Cloud VA Health Care System to ensure patient safety, prevent infection, and comply with regulatory standards, following a Patient Safety Alert issued April 18th, 2024.

• Provide comprehensive inspection of approximately 316 hospital-grade mattresses annually (quantities may vary +/- 5%).

• Inspect mattresses including, but not limited to, Geo Mattress, Comfortrest/Umano, Spann, Hill Rom, and Hovermatts.

• Perform visual inspection of mattress covers for damage (cuts, pinholes, tears, stains, etc.).

• Inspect zippers for proper closure and damage.

• Remove covers (during secondary inspections) to check inner core for staining or odor indicating fluid ingress.

• Conduct fluid ingress tests (e.g., black light, pinless moisture meter).

• Test mattress impermeability per manufacturer recommendations.

• Assess mattress support for adequate pressure redistribution.

• Check structural integrity (sagging, splitting, foam failure).

• Identify and report all failing mattresses.

• Mark failing mattresses with blue duct tape.

• Contractor personnel will be escorted by VA staff; VA staff will remove linens.

• Develop and submit a written inspection checklist for approval within 10 calendar days of award.

• Provide a minimum of 24-hour notice for schedule changes to the VA POC.
