• Registration: Successful offerors must be registered in the System of Award Management (SAM).
• Penalties: False or misleading information is subject to legal penalties under 18 U.S.C. 1001 and 31 U.S.C. 3729-3731, 18 U.S.C. 287.
• Representation Section: Requires the offeror to check a box indicating whether they represent that their offer does or does not meet the definition of an "Indian Economic Enterprise" with "Indian Ownership."
• Information Fields: Includes spaces for "Name of Federally Recognized Tribal Entity," "Name of the 51% owner of IEE Firm," "Business Name," "DUNS Number," "Certifying Signature," and "Print Name."

This form is critical for any bidder seeking to qualify for contracts or set-asides under the Buy Indian Act. Bidders must accurately complete this self-certification to demonstrate their eligibility. Failure to meet the criteria or provide accurate information can lead to disqualification or penalties.

Solicitation / RFP / RFQ

This solicitation is a request for quote (RFQ) for a contractor to provide Gas Range Stoves, Microwaves, and Refrigerators to the Facility Maintenance Department at Chinle Comprehensive Health Care Facility (CCHCF). The acquisition is set aside for Indian Small Business Economic Enterprises.

• Gas Range Stoves (13 units)

• Supplies must be in strict accordance with attached specifications.

• Brand name or equal descriptions will be accepted.

• Contract type will be firm-fixed-priced.

• Delivery is required by November 30, 2026.

• Offers must be submitted via email to teola.autaubo@ihs.gov by May 8, 2026, 1700 MT.

• Quotes must include solicitation number, quoter's name/address/phone, UEI, EFT indicator, and information to evaluate factors per 52.212-2.

• Submission format requires PDF files, with clear labeling for Volume 1 (Administrative Documents, including signed SF-1449 and IHS IEE Representation Form), Volume 2 (Technical Capability/Specifications), and Volume 3 (Price).

• Pricing information should not be included in Volumes 2 or 3.

• Questions are due 10 days before the offer due date.

• Evaluation will be based on Lowest Price Technically Acceptable (LPTA).

• This acquisition is set aside for Indian Small Business Economic Enterprises.
