This document is the VA Notice of Limitations on Subcontracting—Certificate of Compliance for Services and Construction, a mandatory certification for offerors. It outlines the requirements for complying with subcontracting limitations as stipulated in the solicitation and resultant contract.

• Services: Contractors must ensure that no more than 50% of the contract amount is paid to firms that are not certified Service-Disabled Veteran-Owned Small Businesses (SDVOSBs) or Veteran-Owned Small Businesses (VOSBs).
• General Construction: For general construction contracts, no more than 85% of the contract amount can be paid to non-certified SDVOSBs or VOSBs. Costs of materials are excluded.

• Certification Acknowledgement: Offerors acknowledge that false certifications are subject to penalties under Title 18, United States Code, Section 1001.
• Compliance Verification: The VA may require documentation to verify compliance during contract performance and upon completion.
• Cooperation: Offerors agree to cooperate and provide requested documents for compliance determination.

This form requires bidders to certify their understanding and intent to comply with specific subcontracting limitations based on the contract type (Services, General Construction, or Special Trade Construction). Failure to complete, sign, and return this certification with the offer will render the response ineligible for award. Bidders must carefully review the percentages and definitions related to SDVOSB/VOSB subcontracting to ensure compliance.

Solicitation / RFP / RFQ

This is a combined synopsis/solicitation for commercial products and services for preventative maintenance on GE Physiologic Monitor E-SC(Ai)O(V)(X) Modules. The procurement is being conducted in accordance with FAR subpart 12.202.

• Furnish all materials for annual or quad-annual preventative maintenance (PM) of GE E-SC(Ai)O(V)(X) Physiologic Monitor Modules.

• Ensure equipment functions in conformance with the latest published editions of NFPA-99, UL, OSHA, AAMI, TJC, and VA standards.

• Ensure equipment performance/specifications meet or exceed original acceptance levels.

• Provide replacements for non-functional or non-conformant materials at no additional cost.

• Unscheduled maintenance: Contractor's Field Service Engineer (FSE) must respond via phone within two (2) hours of telephonic notification, 24 hours a day.

• Scheduled maintenance: Timely shipment of parts to ensure equipment availability for maintenance.

• Parts: Contractor shall furnish and replace all parts, ensuring ready access to unique/high-mortality parts. All parts must be new or OEM certified rebuilt (with CO/COR approval for rebuilt parts), compatible with existing equipment, of current manufacture, and perform identically to original specifications.

• Documentation: Detailed descriptions of parts numbers, shipping information, quantity, and other conformance-related information.

• Records Management: Compliance with all applicable records management laws, regulations, and NARA policies, including safeguarding of Federal records and reporting any unauthorized disclosures.

• Base year period of performance: 5/1/2026 - 4/30/2027.

• Four (4) option years.

• Place of Performance: Clement J. Zablocki VA Medical Center, 5000 West National Ave, Milwaukee, WI 53295.

• Hours of Coverage: Monday through Friday, 7:00 AM to 5:00 PM, excluding federal holidays.

• Response Date/Time: March 3, 2026, 09:00 AM Central Time, Chicago, USA.

• Required Quote Information: Solicitation number, Offeror name/address/phone, Unique Entity Identifier (UEI), information for technical compliance, completed representations and certifications (via SAM.gov), and a statement regarding acceptance of terms and conditions.

• Evaluation Factors: Technical, Past Performance, and Price. Veterans involvement may be used in case of a tie.

• Award Basis: Comparative evaluation of quotes based on the stated factors, resulting in the quote most favorable to the Government.

• This solicitation is set-aside for small business.
