Statement of Work (SOW) / Performance Work Statement (PWS)

To provide comprehensive full-service maintenance, routine, and emergency repair services for government-owned sterilizers, washers, dryers, and other related equipment at the Salt Lake City VA Medical Center.

• Maintenance & Repair: Ensure all equipment functions in conformance with original manufacturer specifications and relevant industry standards (NFPA-99, UL, OSHA, VA manuals).

• Uptime Requirement: Equipment must be operable and available 95% of normal operating hours (8:00 AM - 5:00 PM, Monday-Friday, excluding national holidays). Downtime is computed from notification during normal work hours; scheduled maintenance is excluded.

• Service Hours: All service/repairs shall be performed during normal working hours unless requested or approved by the COR. Work outside normal hours at COR's request will be billed at prevailing hourly rates.

• Unscheduled Maintenance (Emergency Repairs): Provide 24/7 priority technical support and remote monitoring. Field Service Engineer (FSE) must respond within 30 minutes of telephone notification. If unresolved, FSE must arrive on-site within 4 hours of notification.

• Scheduled Maintenance: Perform semi-annual Preventive Maintenance (PM) services on all equipment. An outline of PM procedures and schedule will be provided within two weeks of award. PM includes cleaning, diagnostics, calibration, lubrication, remedial maintenance, parts replacement, electrical/mechanical inspections, annual chamber cleaning for steam sterilizers, and documentation.

• Parts: Furnish and replace parts as needed to meet uptime requirements. Parts must be equal to or exceed OEM specifications. Rebuilt or used parts are not permitted.

• Tools & Manuals: Supply all necessary tools, test equipment, service manuals, and diagnostic software.

• Operational Uptime: 95% of normal operating hours, computed monthly. Failure to meet may subject the contractor to default actions.

• Emergency Response: 30-minute phone response, 4-hour on-site arrival for unresolved issues.
