Statement of Work (SOW) / Performance Work Statement (PWS)

This document outlines the requirements for providing support and maintenance services for the existing IVIS Lumina S5 Imaging System located at the Iowa City VA Health Care System.

• Service Plan: Provide the Revvity “Signature” Service plan.

• Preventative Maintenance: Conduct one annual Preventative Maintenance.

• Support: Offer priority remote support during normal business hours (8:00 AM – 4:30 PM CT) and unlimited repairs.

• Emergency Repairs: Provide emergency on-site repair services within 3 business days, including all travel, labor, and parts.

• Documentation: Submit a service report detailing all repair services performed upon completion.

• Response Time: Respond to service reports within 2 business days (Monday through Friday) via phone or email.

• Notification: Notify the COR via email or phone before commencing services during normal duty hours. Arrange access with the COR for services outside normal hours.

• Safety: Exchange hazard communication information before repairs. Comply with OSHA lockout/tag-out standards when performing maintenance.

• Service Reports: Provide detailed service reports to the COR upon completion, including cause of malfunction, corrective action, time to complete, and parts replaced. If all information is not immediately available, an initial report is due within five days, with the balance due within ten days.

• Parts & Software: Use only Original Equipment Manufacturer (OEM) new or OEM refurbished repair parts. Furnish all software updates/patches issued by the manufacturer.

• Personnel: Service representatives must be factory-trained with a minimum of two years of experience on the specific medical device.

• Base Period: July 1, 2026 – June 30, 2027

• Records Management: Contractors must comply with all applicable records management laws, regulations, and NARA policies.
