This questionnaire requests details on company identification, market delivery models (direct sales, VARs/Resellers), operational and legal responsibilities, Service Level Agreement (SLA) management, pricing models (Public/Commercial Rates, Private Offers, Enterprise Discount Programs, Tiered Volume Discounts), support services, and specific Cloud Service Offerings (CSOs).
