This form is to be completed by entities that have previously contracted with the offeror. Bidders should be aware that their past performance will be evaluated using this questionnaire. The information gathered will directly influence the Government's assessment of the offeror's capability to successfully perform the work required by the solicitation. Bidders should ensure their past performance is well-documented and that their references are prepared to provide comprehensive and positive feedback.

Solicitation / RFP / RFQ

This solicitation is for a Marriage Enrichment Retreat (MER) program sponsored by the United States Navy Chief of Chaplains. The MER is a three-day, two-night retreat designed to equip married military couples with practical skills and insight to enhance their relationships. The retreat requires a quiet, private, and secure environment conducive to reflection and personal interaction.

• Provide a 4-Star Rated (or higher) venue and supporting services for one (1) three-day, two-night MER.

• Facility must be a resort, hotel, boutique hotel, or inn located within 1.5 miles of Lake Geneva, Wisconsin, with access to the lake.

• Provide accommodations for up to 43 individuals (20 couples and 3 CNRMA CREDO staff) in 23 guest rooms.

• Offer training/conference room, private dining room, on-site staff, recreational amenities, and an on-site full-service restaurant.

• Provide food and beverage services, including five meals (buffet dinners and breakfasts, voucher-based date night meal), three snack services, and beverage service during training hours.

• Accommodate special dietary requirements.

• Provide complimentary Wi-Fi, ADA-compliant elevator, and ice machines.

• Facility must comply with ADA and Hotel and Motel Fire Safety Act.

• Pricing Arrangement: Firm Fixed Price.

• Period of Performance: 28 August 2026 to 30 August 2026 (Primary Target Date); 21-23 August 2026 (Alternate Target Date).

• Estimated value: USD 40,000,000.00 (Size Standard for NAICS 721110).

• Offer Due Date: 09 June 2026, 10:00 AM Local Time.

• Proposals will be evaluated on an Acceptable/Unacceptable basis for Technical Factor I, and based on past performance and price.

• Technical submission must demonstrate meeting minimum qualifications outlined in the PWS.

• Past performance requires submission of up to two (2) most relevant contracts within the last five (5) years.

• Price quote must include total price and a complete breakdown of cost.

• The acquisition is set aside for Small Business.

• NAICS Code: 721110 (Hotels and Motels).

• The Government reserves the right to cancel or reschedule the MER at no additional cost.

• All changes must be finalized through a written contract modification.

• Payment will be made via DFAS Electronic Funds Transfer (EFT) utilizing the Wide Area Work Flow (WAWF) System.
