• Representation: A checkbox section where the offeror self-certifies whether they "does" or "does not" meet the definition of an "Indian Economic Enterprise" with "Indian Ownership" under the Buy Indian Act.
• Name of Federally Recognized Tribal Entity: Field for the name of the tribal entity, if applicable.
• Name of the 51% owner of IEE Firm: Field for the name of the primary owner.
• Business Name: Field for the business name.
• Unique Entity Identifier (UEI) Number: Field for the business's UEI.
• Certifying Signature: Field for the signature of the authorized representative.
• Print Name: Field for the printed name of the signatory.

Bidders must complete and submit this form as part of their offer if the solicitation or RFI requires it. It is crucial for determining eligibility for contracts awarded under the Buy Indian Act. Failure to meet the definition of an "Indian Economic Enterprise" or providing false information can lead to disqualification or penalties. Offerors must also be registered in the System of Award Management (SAM).

Statement of Work (SOW) / Performance Work Statement (PWS)

To provide a comprehensive, web-based Materials Management and Electronic Requisition (E-Ordering) Software System to support the Property and Supply Department at Cheyenne River Health Center. The system aims to manage inventory, ordering, and tracking of office and medical supplies for direct patient care and administrative functions.

• Provide a web-based software solution.

• Include all licensing, maintenance, technical support, and software updates.

• Support the Property and Supply Department in managing inventory, ordering, and tracking of supplies.

• Inventory management and tracking of medical and office supplies.

• Electronic requisitioning and order processing.

• Monitoring of supply usage and consumption trends.

• Automated tracking of stock levels and reorder points.

• Historical data retention for reporting and audit purposes.

• Support for multiple departments and the service unit.

• Reporting capabilities for operational and compliance needs.

• Integrated materials management and e-ordering in a single platform.

• Real-time inventory tracking and visibility.

• Automated requisition and approval workflow.

• Ability to track supply usage by department and cost center.

• Historical data storage and reporting functionality.

• Reliable system availability with minimal downtime.

• Secure, role-based user access controls.

• Scalable to support facility-wide operations.

• Capability to support direct patient care supply needs without disruption.

• IT requirements must align with Indian Health Service safety and security guidelines.

• Vendor-provided technical support, including routine maintenance, updates, and user assistance.

• Ongoing maintenance and technical support.

1 year from the date of award.

Firm-fixed-price, billed on a monthly basis for licensing and support services.

Software maintenance, updates, and support services are included for the duration of the contract at no additional cost.

The system will be delivered and maintained as an online service. Required hardware will be provided at no cost upon program installation to authorized users within the Service Unit.
