This pricing sheet is critical for bidders to understand the specific items being procured and to formulate their price quotes. Bidders must provide their proposed item prices for each line item, which will be used to calculate the total price for the solicitation.

Statement of Work (SOW)

This Statement of Work (SOW) outlines the requirements for the purchase and delivery of 456 mattresses and 912 protective mattress encasements to 15 separate Missile Alert Facilities (MAFs).

• Purchase and delivery of 441 Standard Twin XL mattresses and 15 custom-sized mattresses.

• Purchase and delivery of 912 protective mattress encasements (two per mattress).

• Delivery to 15 specified facilities, with locations detailed in Appendix A and distances in Appendix B.

• Pickup and disposal of old mattresses from the 15 facilities within 5 days of new mattress availability.

• Full contract performance, including delivery and pickup, must be completed within 120 days of the contract date.

• New mattresses must be Beautyrest Silver BRS900 Medium Firm or equal, meeting specific dimensions, comfort level, and core support requirements.

• Protective encasements must be laboratory-certified waterproof, resistant to bed bugs, dust mites, allergens, mold, and liquids, and meet ASTM F1670 and ASTM F1671 standards.

• Encasements must have a zippered closure that fully encloses the mattress on all six sides.

• Delivery to 15 separate facilities across various locations (see Appendix A).

• Full performance (delivery and pickup) to be completed within 120 days of contract award.

• Contractor employees require pre-approval and must be on a current Entry Authorization List (EAL) at least two weeks prior to delivery/pickup. This involves background checks and submission of personal information.

• Contractor vehicles must also be on the EAL.

• Employees are prohibited from bringing electronic devices and weapons on-site.

• Contractor must comply with all applicable federal, state, and local environmental regulations for disposal of old mattresses.

• Services are to be performed between 0800 and 1700, excluding Federal Holidays. Deviations require coordination with the Government POC.

• Contractor is responsible for coordinating delivery and pickup with the Government POC 72 hours in advance.

• All products are covered by the manufacturer's standard commercial warranty.
