Statement of Work (SOW) / Performance Work Statement (PWS)

To acquire linen laundering services for 355 Medical Group (MDG) personnel, managing all off-site aspects and ensuring timely, accurate weekly deliveries.

• Provide all equipment, materials, transportation, personnel, labor, supervision, and management for linen laundering, delivery, and pick-up services.

• Services support patient care programs and base veterinary services for the 355 MDG at Davis-Monthan AFB, AZ.

• Clean linen must be protected from contamination during transit, delivered in enclosed, sealed trucks.

• Soiled linen will be picked up from the dirty linen room.

• Laundering services must be performed at a contractor-owned and operated facility, complying with infection control standards and Healthcare Laundry Accreditation Council (HLAC) standards.

• Specific items include operating shirts, trousers, jackets, pediatric garments, veterinarian clinic linen, and contaminated laundry.

• Clean linen must be free of soil, foreign objects, dampness, offensive odors, mildew, chemical residue, bacteria, fungi, and microorganisms.

• Laundry must be bundled, wrapped, and sealed in clear plastic wrap.

• Pick-up and delivery between 8:30 a.m. and 10:30 a.m. on Tuesdays.

• 95% compliance quarterly for pick-up and delivery schedule.

• Weight of delivered linen must match weights on the delivery ticket (no more than 2 defects per month, not to exceed 5 pounds).

• Cart care and maintenance: standard commercial bulk plastic laundry carts in good operating condition, cleaned and treated with a germicidal agent.

• Rejected laundry must be re-performed and delivered by the next scheduled delivery day at no cost to the Government (90% compliance quarterly).

• Contractor's plant facility must meet inspection standards (100% compliance).

• Deliveries primarily to the 355 MDG facility on DMAFB.

• All laundering services at contractor-owned and operated facilities.

• Contractor must provide a Mission Essential Plan and Contractor Mission Essential Employee Identification.

• Compliance with applicable Federal, State, and local laws, codes, and regulations.

• Personnel must possess valid driver's licenses and comply with base traffic regulations.

• Antiterrorism Awareness Training is required for contractor employees.

• Contractor employees must report suspicious activity and adhere to incident notification procedures.

• Contractor employees are subject to random personnel and vehicle searches.

• Possession of weapons, firearms, or ammunition is prohibited on base.

• Contractor personnel must limit travel on the installation to specific areas.

• Contractor must ensure employees understand and comply with Privacy Act and DoD Freedom of Information Act Program requirements.

• Multiple drivers must be available to ensure service continuity during medical or emergency absences.

• Contractor employees must present a professional, conservative, and neat appearance.

• Open, professional, and respectful communication with Government personnel is required.

• Contractor employees are prohibited from endangering persons or property, or exhibiting conduct inconsistent with professional conduct or military security.

• Smoking is restricted to designated areas.

• Compliance with all applicable safety regulations, including OSHA.

• Contractor shall provide necessary personal protective clothing and equipment.

• Government will provide plastic bags for soiled linen collection and commercial grade scales.

• Government will provide security forces and fire protection services; contractor must call for medical emergencies.
