Scope of Work
The contractor will provide licensed facilities, equipment, personnel, and services for the embalming, restoration, preparation, care, and disposition of remains. This includes transportation, supplies, death certificates, caskets, and urns, all in accordance with the detailed Performance Work Statement (PWS). Services must be performed promptly, generally within 24 hours of notification, within a four-hour radius of Keesler AFB. Cremation services are limited to subsequent portions or retained organs.

Submission & Evaluation
Offerors must submit quotes electronically in three volumes: Price, Technical, and Site Inspection. Key attachments include the Price List (Attachment 2) and Technical Information (Attachment 3). Evaluation will consider Price, Technical capability, and Site Inspection, with Technical and Site Inspection weighted equally and approximately equal to cost/price. An "Acceptable" rating is required in all evaluation parts for award. Compliance with the Service Contract Act Wage Determination (Attachment 7) is mandatory. Offerors must acknowledge Amendment 0001.
