Request for Information (RFI)

This document is a Request for Information (RFI) to identify qualified contractors capable of providing Durable Medical Equipment (DME) storage, delivery, installation, repair, and pick-up services for the Sioux Falls VA Health Care System. The government is seeking information on potential contractors and their socio-economic classifications under NAICS code 621610 (Home Health Care Services).

• Storage, delivery, installation, repair, and pick-up of VA-owned durable medical equipment.

• Delivery and pick-up of recovered government-owned equipment (e.g., wheelchairs, power chairs, scooters, hospital beds).

• Services required within three working days of notification, or within 24 hours for emergency services.

• Services are generally required during work hours (Monday-Friday, 8:00 a.m. to 4:30 p.m.), with provisions for non-work hours and Federal Holidays.

• Delivery locations include the Sioux Falls VA Medical Center and surrounding counties in South Dakota, Minnesota, Iowa, and Nebraska.

• Contractors must be regularly established, financially responsible businesses with necessary equipment, personnel, and facilities.

• Must meet State and City codes for operations.

• Must be Joint Commission accredited or accredited by an equivalent national organization, and maintain accreditation throughout the contract period.

• Strict inventory control and record-keeping for Government-Owned Property (GOP) are required.

• All equipment must be cleaned and disinfected according to manufacturer guidelines and Joint Commission standards.

• Personnel must have current driver's licenses, undergo criminal background checks, and hold applicable certifications.

• Contractor must accept government purchase cards (VISA) and possess Level II purchase card capability.

• DME delivery within 72 hours of task order receipt (unless otherwise arranged).

• Unneeded DME pick-up within 48 hours of task order receipt.

• Weekly inventory lists to be provided to the COR.

• Delivery certifications to be received by PSAS within 24 hours after service.

• DME repairs and returns within 48 hours of notification (unless non-stocked parts are needed).

• Responses are due by June 19, 2026, 3:30 PM CT.

This document is the VA Notice of Limitations on Subcontracting—Certificate of Compliance for Services and Construction (JAN 2023) (DEVIATION). It requires offerors to certify their compliance with subcontracting limitations if awarded a contract.

• Services: For service contracts (excluding construction), contractors must ensure no more than 50% of the contract value is paid to firms that are not certified SDVOSBs or VOSBs. Work further subcontracted by a similarly situated certified SDVOSB/VOSB subcontractor counts towards this limit.
• General Construction: For general construction contracts, no more than 85% of the contract value can be paid to firms that are not certified SDVOSBs or VOSBs.
• Special Trade Construction: For special trade construction contracts, no more than 75% of the contract value can be paid to firms that are not certified SDVOSBs or VOSBs.
• Certification Requirement: Offerors must complete, sign, and return this certification with their bid, quotation, or proposal. Failure to do so will render the offer ineligible for award.
• Penalties: False certifications are subject to criminal, civil, or administrative penalties, including prosecution under Title 18, United States Code, Section 1001.
• Compliance Verification: Offerors acknowledge that they may be required to provide documentation (e.g., invoices, subcontracts) to verify compliance with subcontracting limitations at any time during contract performance or upon completion.
