Statement of Work (SOW) / Performance Work Statement (PWS)

This Performance Work Statement (PWS) outlines the requirements for Non-Personal Service Contract for Registered Nurse (RN) Operating Room/Recovery (OR/PACU) nursing services to support the Indian Health Service (IHS) at the Northern Navajo Medical Center (NNMC) and associated Navajo Area facilities.

• Provide perioperative nursing support for scheduled and add-on cases in the Operating Room (OR), Pre-Operative Area, Day Surgery Area, Post Anesthesia Recovery, and Endoscopy Room.

• Duties include patient admission, pre-procedure preparation, nursing assessments, IV therapy, patient/family education, coordination with surgical teams, circulating duties in the OR, maintaining sterile fields, assisting with procedures, post-anesthesia care, and discharge readiness evaluation.

• Services are to be performed at hospitals, clinics, and other healthcare facilities within the Shiprock Service Unit, with the primary location being Northern Navajo Medical Center in Shiprock, NM.

• Performance will be evaluated based on standards defined in the PWS, including patient care, compliance, cultural sensitivity, documentation, and professionalism.

• Acceptable Quality Levels (AQL) require ratings of "Fully Successful" or "Exceptional" on the Peer Review NAO Provider Exit Profile Form.

• Performance is subject to direct observation, periodic inspection, and analysis of contractor progress reports.

• Period of Performance: 1 Base Period + 4 Option Periods, each 6 months in duration.

• Place of Performance: Shiprock Service Unit, primarily Northern Navajo Medical Center, Shiprock, NM.

• Contractor must possess a current, valid, unrestricted nursing license.

• Required certifications include Basic Life Support (BLS), Advanced Cardiac Life Support (ACLS), and Pediatric Cardiac Life Support (PALS).

• Medical evaluation, specific immunizations (Rubella, Mumps, Rubeola, Hep B, Varicella, Tdap, TB testing, Influenza), and background checks are required.

• Contractors must demonstrate cultural awareness and respect for American Indian and Alaska Native culture.

• Adherence to HIPAA, privacy laws, and IHS IT security policies is mandatory.

• Nurses must attend mandatory orientations and training specified by the government.

• Contractor is responsible for managing its own leave policies and ensuring adequate staffing levels.

• Contractors must wear a government-issued identification badge.

Solicitation / RFP / RFQ

This document is a combined synopsis/solicitation for commercial products or services, issued as a Request for Quotation (RFQ) under solicitation number 75H71026Q00174. It constitutes the only solicitation, and a separate written solicitation will not be issued. The acquisition is set aside for Indian Small Business Economic Enterprises (ISBEE). The solicitation outlines a requirement for registered nurses. • Volume III: Pricing Information: Provides detailed pricing aligned with the technical approach, including a budget narrative and breakdown of hourly rates. All Base and Option CLINs must be priced.

Submissions must be in PDF format, with each volume as a separate file. Quotes failing to address all volumes will be marked "Non-Responsive."

Evaluation will be based on technical capability and approach, key personnel qualifications, capability to recruit and retain personnel, and price. The award will be made to the responsible offeror whose offer is most advantageous to the Government.

This acquisition is an Indian Small Business Economic Enterprise (ISBEE) set aside.
