• Delivery Questions: Information on production limitations and potential minimum delivery order quantities or dollar values.
• Additional Information: An open section for any other relevant input.

Potential bidders are requested to complete this survey to inform the government's acquisition strategy and solicitation development. The responses will help DLA understand market capabilities, pricing, contract preferences, and potential challenges. Bidders are asked to return the survey within 5 business days of receipt.

This spreadsheet provides a list of National Stock Numbers (NSNs) along with their nomenclature, CAGE codes, approved sources, part numbers, and other relevant government-supplied information. It is intended for contractors to fill in their pricing and delivery information.

• NSN: National Stock Number for the item.
• Nomenclature: Description of the item.
• CAGE: Commercial and Government Entity code of the approved source.
• Approved Company/Source: Name of the approved supplier.
• Part Number / Drawing Number: Specific part or drawing identifier.
• AMS / AMSC: Item management codes.
• NAICS Code: North American Industry Classification System code.
• Estimated Annual Demand Quantity (ADQ): Government's estimate of annual demand.
• Quantity Price Break Ranges: Fields for contractors to enter their proposed quantity ranges.
• Enter Your Various Quantity Ranges: Fields for contractors to input their pricing for the specified quantity ranges.
• Estimated Delivery in days: Contractor to provide estimated delivery time.
• Maximum Qty. / Maximum Qty. Delivery: Fields related to maximum order quantities and their delivery.
• Does a Minimum Quantity Apply: Yes/No indicator for minimum order quantities.
• Are these items Quantity Sensitive: Yes/No indicator for quantity sensitivity.
• Are you the actual Manufacturer of this NSN: Yes/No indicator.
• The Name of the company that makes this item for you and their CAGE Code: Fields to provide information if the contractor is not the manufacturer.

This document is crucial for bidders to understand the specific items being procured, their estimated demand, and to provide their pricing and delivery terms. Bidders must accurately complete the 'Contractor Fill-in Information' sections for the relevant NSNs to submit a responsive quote or proposal.
