This is a combined synopsis/solicitation for commercial products or services prepared in accordance with FAR Part 12. The government is seeking quotes for Occupational Therapy Services for the John F. Kennedy Day School located in Whiteriver, Arizona. This announcement constitutes the only solicitation, and separate written solicitations will not be issued.

• Evaluation Factors: Quotes will be evaluated based on Capabilities, Key Personnel, Past Performance, and Price. The government will award to the responsible quote whose offer is most advantageous, considering price and other factors.

• Quote Format: Submit price volume, SF-1449, and technical volume (addressing capabilities, key personnel, and past performance) in PDF format. Do not use zipped files.

• This acquisition is UNRESTRICTED.

• Prospective contractors must review Section E (Instructions to Offerors) and Section 52.212-2 (Evaluation) in their entirety.

Statement of Work (SOW) / Performance Work Statement (PWS)

To ensure the provision of Occupational Therapy Services to special education students identified or referred for services at John F. Kennedy Day School, in accordance with P.L. 108-446 (Individuals with Disabilities Education Improvement Act of 2004) and Individual Student Individualized Education Plans (IEPs).

• Provide direct in-person Occupational Therapy (OT) to students.

• Conduct consultations with staff.

• Perform screenings and evaluation reports.

• Provide instructions/directions to teaching assistants.

• Participate in IEP meetings.

• Engage in parent contact regarding concerns.

• Enter quarterly progress reports.

• Utilize the NASIS system for entering evaluation summaries, present levels, OT goals, and OT services.

• Maintain a direct service log for each student and visit.

• Meet with the Special Education Department at the contract's start and periodically for updates.

• Services to be provided for approximately 6 students needing OT on a bi-weekly basis.

• Contractor will be on-site for 2 days per month.

• Services to be provided from August 2025 through May 2026, with a 4-year option.

• Specific tasks include observing, screening, evaluating students, documenting referrals, attending multidisciplinary meetings, developing/implementing therapy plans, maintaining service logs, providing supervision to teaching assistants, identifying equipment needs, completing reevaluations and initial evaluations per BIE timelines, submitting written evaluation reports, and submitting written progress reports at the end of each school quarter.

• Provide training and support to staff and parents as needed.

• Period of Performance: August 2025 through May 2026 (with a 4-Year Option).

• Place of Performance: John F. Kennedy Day School, Cedar Creek, Arizona (on the Fort Apache Indian Reservation).

• The Occupational Therapist must have experience working with school-age children, staff, parents, and/or guardians.

• Must possess a Master's Degree in Occupational Therapy.

• Must be Certified Licensed in the State of Arizona for Occupational Therapy.

• Must hold National Certification in Occupational Therapy.

• The service provider is to submit all documentation and reports to the Special Education Department, collaborate with the Special Education Coordinator to establish a schedule, and meet with the Special Education Department on the first day of work to organize service provision structure and procedures.
