- A Capability Statement with active CAGE code, POC, demonstrated experience with CT systems, and details on subcontractors/teaming.
- Quoted prices in the specified format.
- Acknowledgement and signed copy of Amendment P00001.
- Foreign entities may need to complete IRS Form W-14. Evaluation will be based on Technically Approved, Supplier Performance Risk System (SPRS), and Lowest Price. Quotes must be technically acceptable, have an acceptable SPRS rating, and be the lowest price.
