• A004: Transition In and Initial Staffing Plan: Requires a plan detailing personnel, labor categories, status of offers to incumbent personnel, recruitment, transition milestones, roles, and government actions needed. Due 5 days after award.
• A005: Sustainment Staffing Plan: Requires a plan documenting current staff, how staffing will be maintained, strategies for filling vacancies, and contractor training needs. Due 90 days after award.
• A006: Travel Reports: Requires monthly reports detailing traveler name, reason for travel, program supported, estimated and actual costs, and dates of travel. Due 5 business days after individual travel concludes.
• A007: Annual Master Travel Log: Requires an annual log of all contractor travel, including traveler name, reason, program supported, estimated/actual costs, and dates, plus recommendations for cost reduction. Due two weeks prior to the end of the period of performance.
• A008: Ad Hoc Presentation Materials: The COR may request technical reports, presentation materials, and other items as needed, with specified format and content.
• A009: Quality Control Plan: Requires a plan detailing the organizational chart, personnel roles, problem resolution, and correction of deficiencies. Due 10 days after award.
• A010: Non-Key Personnel Resumes: Requires submission of resumes for non-key personnel, not exceeding 2 pages each, indicating employment status. Due 3 business days after award.
• A011: Non-Key Personnel Matrix: Requires a matrix correlating non-key personnel to PWS labor categories, demonstrating how they meet requirements and linking to resumes. Submitted in Excel. Due 3 business days after award.
• A012: Quarterly Contract Management Review (QCMR) Briefing Slides: Requires quarterly briefing slides covering contract status, performance issues, financial status, out-of-scope work, and lessons learned. Due 5 days prior to the QCMR meeting.
• A013: Quarterly Contract Management Review (QCMR) Meeting Minutes: Requires quarterly meeting minutes including attendees, discussions, and action items, with briefing slides attached. Due 30 days after the QCMR meeting.
• A014: System Security Plan and Associated Plans of Action: Requires a plan for the contractor's internal unclassified information system, submitted with encryption if necessary. Due 30 days after award.
• A015: Government Property Inventory Report: Requires an annual report of government property. Due June 30 or 30 days prior to the end of the period of performance.

Statement of Work (SOW) / Performance Work Statement (PWS)

To provide technical, program management, and financial analyst support to the Office of Naval Research (ONR) Code 011 Strategy, Policy, Plans, and Assessment (SPPA) and Code 03 Science and Technology (S&T) portfolio.
