Scope of Work
The contractor will provide comprehensive optometry support services on an as-needed basis at the Warm Springs Health and Wellness Center. Services include:

- Screening & Community Services: Head Start, school vision, diabetic retinopathy screenings, liaison with other professionals, vision safety, and community education.
- Treatment & Referrals: Providing or referring treatments, coordinating scheduling with clinic staff, prescribing drugs/diagnostic tests (following IHS formulary), and processing referrals via the Patient Referred Care (PRC) process.
- Documentation & Coordination: Recording patient care in electronic health records (IHS-EHR, Microsoft Office, Outlook), coordinating with allied health professionals, and ensuring quality of care.
The contractor is not guaranteed a minimum number of hours or patient encounters. Services will be requested and scheduled by the Government based on clinical need.

Required Qualifications
Contractors must possess:
- A Doctor of Optometry degree from an approved institution.
- Current Board Certification in Optometry.
- A permanent, full, and unrestricted license to practice optometry in a U.S. state or territory, permitting independent practice.
- DEA registration is recommended.
- Compliance with the Indian Child Protection and Family Violence Prevention Act (no unsupervised contact with Indian children until background check is complete).

Award will be made to the responsible Respondent whose quote represents the best value to the Government, considering price and non-price factors. The Government may award to other than the lowest-priced offeror, prioritizing the ability to provide timely and reliable services.
- Factor 1 – Demonstrated Prior Experience: Evaluated on experience in comparable optometry services, especially in remote or rural healthcare environments with limited housing.
- Factor 2 – Price: Evaluated based on the CLIN structure in Attachment No. 1, covering all costs including labor, services, supervision, and incidental costs. Travel costs are not reimbursed separately.
