Amendment / Modification

This amendment modifies Solicitation number 36C25626Q0402 for Patient Lift Maintenance.

Key changes and clarifications include:

• Price Cost Schedule & Invoicing: The price cost schedule should be structured for 750 systems. Invoicing terms are requested to be monthly in arrears, billed only for serviced lifts, and can cover up to 750 lifts. Bidders can rearrange their response as appropriate.

• 3G Program Pricing: To qualify for 3G Program pricing, the Statement of Work (SOW) must explicitly include "3G Program," or the Price/Cost Schedule must clearly identify this. Bidders must provide a response for the 3G Program.

• Section 2.3.1.6 Clarification: The requirement to "inspect and replace... electrical wiring and cables" is clarified to apply only to wiring and cables associated with the Guldmann patient lift equipment and related system components, not general facility electrical systems.

• Room Availability: The SOW will state, "Dependent on patient occupancy, the facility will make best effort to have at least 5 rooms available daily."

• Digital Reports: Digital reports are due every Monday after contract initiation. If a Monday is a holiday, the report is due the next business day. This excludes the week between Christmas Eve and New Year's Day. The SOW will read, "once the day of submittal falls on a holiday, report will be due to next business day or coordinate with COR to provide."

• Price Cost Schedule structure and invoicing terms.

• Statement of Work (SOW) regarding 3G Program inclusion, electrical work scope, room availability, and digital report submission.

Bidders need to ensure their proposals and pricing reflect the updated requirements for the 3G Program, invoicing, and the specific scope of electrical work. The SOW language has been updated or will be updated to reflect these changes. No change to proposal deadlines is indicated.

Solicitation / RFP / RFQ

This solicitation is for Preventative Maintenance and Corrective Maintenance services for Guldmann patient lifts located at the Michael E. DeBakey VA Medical Center (MEDVAMC) in Houston, Texas. The services are required to maintain the equipment in operational condition with minimal malfunctions.

• Perform scheduled preventive maintenance (PM) on up to 750 Guldmann patient lifts.

• Maintenance must be performed in accordance with the latest Original Equipment Manufacturer (OEM) standards, ISO 10535, and Section 4, Conformance Standards.

• Inspections must meet manufacturer requirements, including structural and functional checks.

• Complete a "Corrective and Preventive Maintenance Checklist for Ceiling Mounted Patient Lifts" for all inspected lifts.

• Perform weight tests with a full lifting cycle at maximum rated load capacity.

• Replace any damaged items at no cost to the Government.

• Clean the service area after each completed service.

• Coordinate with Nursing staff to ensure empty rooms are available for work, and notify COR in advance for operations requiring ceiling opening cutting or dust creation.

• Provide documentation of services performed, including date of inspection, serial number, manufacturer's checklist, observations, and corrective actions.

• Deliver testing certification records to COR within one week of certification.

• Notify COR immediately of any damage jeopardizing safety and any required corrective action.

• Take photos of weight tests at 100% capacity.

• Ensure work is approved by COR and certified for twelve (12) months.

• Furnish all labor, parts, tools, supplies, materials, equipment, qualified personnel, supervision, management, and transportation.

• Use new, OEM-certified replacement parts compatible with existing equipment.

• Provide proof of FSE qualifications and OEM training.

• Contract Type: Anticipated Firm Fixed Price.

• Period of Performance: April 01, 2026 – March 31, 2027, with four (4) one-year option periods.
