Statement of Work (SOW) / Performance Work Statement (PWS)

This Performance Work Statement (PWS) outlines the requirements for Non-Personal Services for a General and Child & Adolescent Psychiatrist to support the mission of the Indian Health Service (IHS), specifically within the Navajo Area Indian Health Service (NAIHS).

• Provide outpatient general and child & adolescent psychiatry services.

• Duties include psychiatric assessments, prescribing psychotropic medications, providing psychiatric treatment, and offering consultation/education to clinical staff.

• Services are to be performed at the Four Corners Regional Health Center (FCRHC) in Red Mesa, Arizona, in person and/or virtually via Webex.

• No weekend, holiday, overtime, or on-call responsibilities are required, but the contractor must be reachable by phone on weekdays for urgent patient issues.

• Services must be performed in accordance with competency standards in the Shiprock Service Unit Medical Staff Bylaws.

• Adherence to the highest standards of professional ethics, confidentiality, and patient rights is required.

• All patient contacts must be recorded in the FCRHC Electronic Health Record (EHR) using a Mental Health template.

• Contractor performance will be evaluated based on competency, compliance, patient outcomes, professionalism, credentialing, documentation, and service quality, as detailed in the Performance-Based Matrix.

• A quality assurance committee at FCRHC will monitor the provision of psychiatric care.

• Period of Performance: Part-time from May 1, 2026, through April 30, 2027.

• Place of Performance: Four Corners Regional Health Center (FCRHC), Red Mesa, Arizona.

• Work Schedule: At least four days a month, with clinic hours from 8:00 AM to 4:30 PM (Monday-Friday, except Wednesdays 12:30 PM - 4:30 PM). Additional hours may be approved for mandatory training or urgent services.

• Contractor must possess a Doctor of Medicine or Doctor of Osteopathy degree and be board-certified by the American Board of Psychiatry and Neurology in General and Child & Adolescent psychiatry.

• Experience providing telepsychiatry services is required.

• Must hold a permanent, full, and unrestricted medical license in a U.S. state and possess a federal DEA number.

• Must maintain U.S. citizenship.

• Required immunizations include Rubella, Mumps, Measles, Hepatitis B (or declination), chickenpox, Tetanus Diphtheria, TB test, and current flu vaccination.

• Must read, understand, speak, and write English and demonstrate cultural awareness.

• Basic IT skills are required.

• Mandatory orientations and training must be attended.

• Background checks (Level I and Level II) are required.

• Contractor must be credentialed and have clinical privileges granted by the Shiprock Service Unit Credentials Committee.

• Specific documentation for credentialing is required, including licenses, certifications, and malpractice insurance.

• Contractor must abide by all government regulations, policies, and procedures, including HIPAA and IT security policies.

• Government-furnished items include PPE (except uniforms/lab coats), facility-specific ID badges, and access to the FCRHC EHR via VPN for telepsychiatry. Government vehicles may be provided if required.

• Contractor-furnished items include uniforms, lab coats, and personal medical instruments. For telepsychiatry, the contractor provides the remote workstation, computer, and internet.

This document is the Indian Health Service (IHS) Buy Indian Act Indian Economic Enterprise (IEE) Representation Form. It serves as a self-certification for offerors to confirm they meet the definition of an "Indian Economic Enterprise" as required by the Buy Indian Act. This form is to be submitted in response to a Sources Sought Notice, Request for Information (RFI), or Solicitation.

• Eligibility Requirements: The enterprise must meet the definition of an "Indian Economic Enterprise" at the time of offer, contract award, and throughout the contract performance period. Failure to meet these requirements necessitates immediate written notification to the Contracting Officer.
• Representation: Offerors must indicate whether they do or do not meet the definition of an "Indian Economic Enterprise" with "Indian Ownership" under the Buy Indian Act.
