Scope of Work

- Designing and implementing interventions, and providing teacher consultation.
- Providing daily performance data, assisting with goal writing, and participating in IEP meetings.
- Maintaining student rosters and submitting progress reports.
- Conducting comprehensive occupational and physical therapy evaluations using standardized tools.
- Reviewing and analyzing student medical/educational records.
- Participating in Multidisciplinary Evaluation Team (MET) meetings and attending IEP/MET meetings.
- Submitting quarterly progress notes and therapy logs.
- Contractors must provide their own equipment (e.g., laptops) and supplies.
Contract Details
- Contract Type: Time and Material (including Labor Hour) purchase order.
- Period of Performance: A base year from August 1, 2026, to July 31, 2027, with two one-year option periods extending through July 31, 2029.
