Service Agreement for KARL STORZ medical and surgical endoscopy equipment at Naval Medical Center San Diego

Statement of Work (SOW) / Performance Work Statement (PWS)

To provide comprehensive service and maintenance for government-owned surgical endoscopy equipment located at Naval Medical Center San Diego (NMCSD).

• Provide all services, labor, parts, materials, and equipment for the service and maintenance of surgical endoscopy equipment.

• Service and maintenance must meet or exceed Original Equipment Manufacturer (OEM) specifications.

• All services to be performed at the Contractor's off-site certified repair facility.

• Includes unscheduled corrective repairs and necessary calibrations, adjustments, and preventative checks when equipment is at the facility.

• Contractor may use third-party vendors, but they must be approved in advance by the Contracting Officer (KO) and Technical Representative (TR).

• Personnel performing work must be fully qualified and OEM or equivalent certified.

• Test equipment used for repair certification must have calibration traceable to NIST.

• Field Service Reports (FSR) are required for every completed service event within 72 hours of equipment return.

• Repairs aim to restore equipment to a properly serviceable/operating condition.

• Equipment improvements/modifications require BIOMED written approval.

• Contractor must notify BIOMED of any OEM safety recall notices.

• Original design and functional capabilities shall not be altered without express written authorization from BIOMED.

• Replacement parts must be new or OEM-certified refurbished, meeting OEM specifications.

• Maintain an overall uptime rate of 95% or better for all covered equipment.

• Uptime is defined as any time the equipment is not in transit to/from or at the contractor's facility for repair.

• Uptime is calculated on a rolling 6-month basis.

• Response time for telephone consulting is 24/7. During business hours (0800-1600 PST, M-F), response to a malfunction report must be within four (4) hours.

• Services are performed at the Contractor's off-site certified repair facility.

• Equipment is located at Naval Medical Center San Diego, Building 2, Second Floor, ENT Clinic.

• Primary coordination and shipping point of contact is the Biomedical Repair Division, Building One, Ground Floor.

• Contractor shall maintain general liability insurance of at least $500,000 per occurrence and property damage liability insurance of at least $500,000.

• Shipping materials must arrive at NMCSD within two (2) business days of request.

• All shipping and transportation costs are the responsibility of the Contractor.

• NMCSD employees will not perform maintenance or attempt repairs.

• Contractor must provide a written report for irreparably damaged equipment detailing damage and suspected cause.

Includes a list of specific surgical endoscopy equipment requiring service, such as telescopes, documentation systems, cameras, CCUs, monitors, light cables, scopes, and adaptors.
