The prime offeror is instructed to obtain this certification from any small business subcontractor listed in their proposal that is not registered in SAM. The prime offeror must fill in the Solicitation Number, NAICS code, and size standard before sending it to the subcontractor.

• Instructions for Subcontractor: Guidance on filling out the form, emphasizing that NAICS code and size standard must be pre-filled.
• Solicitation Number: Field for the specific solicitation identifier.

• NAICS Code & Size Standard: Fields to be completed by the prime offeror, defining the scope of work for the subcontractor.
• Socioeconomic Status Certification: Checkboxes for the subcontractor to self-certify as Small Business (SB), Small Disadvantaged Business (SDB), Woman Owned Small Business (WOSB), Veteran Owned Small Business (VOSB), or Service Disabled Veteran Owned Small Business (SDVOSB). Note that 8(a) and HUBZone certifications require SBA certification and SAM registration.
• Official Authorized to Sign: Fields for the subcontractor's authorized representative's name, title, signature, and date.
