Scope of Work

• Provision of compliant biohazardous and pharmaceutical waste containers.
• Manifesting, documentation, and certificates of destruction.
• Annual DOT and hazardous materials training for CRSU personnel.
• Adherence to all applicable federal, state, tribal, and IHS regulations.
• Furnishing all labor, equipment, permits, and supervision, ensuring disposal at properly permitted facilities. Disposal services cannot be subcontracted without Contracting Officer approval.

Submission & Evaluation

• Evaluation Basis: Lowest Price Technically Acceptable (LPTA).
• Evaluation Factors: Technical Acceptability (compliance with requirements) and Price (reasonableness).
• Submission: Offers must incorporate FAR clauses by reference and submit pricing via the provided Price Schedule. Payment requests will be submitted electronically via the Department of Treasury Invoice Processing Platform (IPP).

Key Requirements & Notes

• Compliance: Must comply with DOT, OSHA, EPA, Arizona Administrative Code, IHS guidelines, and CRSU Waste Management Requirements. A Wage Determination (No. 2015-5479, Revision 29) for Arizona County of La Paz is applicable.
• Indian Economic Enterprise (IEE): While the solicitation is a Total Small Business Set-Aside, previous market research indicated a priority for Indian Small Business Economic Enterprises (ISBEEs) under the Buy Indian Act. Offerors may need to complete the IHS Indian Economic Enterprise Representation form (Attachment 1) and be registered in SAM.
