Statement of Work (SOW) / Performance Work Statement (PWS)

This Performance Work Statement (PWS) outlines the requirements for providing Durable Medical Equipment (DME) services to Veteran beneficiaries within Veterans Integrated Service Network (VISN) 4. The services include delivery, installation, set-up, instruction, repair, and pick-up of VA-owned DME.

• The Contractor will provide DME services to Veteran beneficiaries serviced by VISN-04 VA Medical Centers' Prosthetics and Sensory Aid Service (PSAS).

• Services encompass all supplies, materials, equipment, transportation, set-up, labor, supervision, patient education, safety management, and infection control.

• Services are required twenty-four (24) hours a day, seven (7) days a week, including Federal Holidays.

• The Contractor must be staffed and stocked to meet contract terms for the duration of the performance period.

• The Contractor must be capable of performing services within the geographic area of any major site or catchment area quoted.

• The contract period includes a Base Year (10/31/2026 – 10/30/2027) and four Option Years (through 10/30/2031).

• Services will be provided to Veteran beneficiaries in VISN-04 geographical areas, with specific catchment areas detailed in ATTACHMENT 1.

• Contract Type: Firm-Fixed-Price Indefinite Delivery Indefinite Quantity (IDIQ).

• Ordering: Task orders will be placed by PSAS representatives or Purchasing Agents via email, facsimile, or other electronic formats.

• Personnel: Contractor personnel must be clean, present a neat appearance, and have manufacturer-specific training and competency certification for DME brands.

• Certifications/Standards: Contractor must meet or exceed Joint Commission (JC) standards for Home Care, even if not JC accredited.

• Infection Control: Contractor must comply with infection control, tuberculosis surveillance, and infectious disease policies. Seasonal flu immunization is required for healthcare personnel.

• Emergencies: Contractor must have a 24-hour communication system and provide emergency services within four (4) hours of notification.

• Data Security: Contractor must comply with VA Information Security requirements and HIPAA.

• Reporting: Contractor must provide various reports including Quality Control Program (QCP) summaries, inventory reports, timeliness reports, and Joint Commission compliance reports.

• Performance Standards: Performance will be assessed based on criteria such as hours of operation, delivery timeliness, accuracy of documentation, and adherence to Joint Commission Standards, with specific Acceptable Quality Levels (AQLs) defined.
