Scope of Work
The contractor will provide comprehensive repair services for endoscopes used in the Phoenix Indian Medical Center (PIMC) Operating Room. Services include diagnostics, repair, replacement of parts, shipping, labor, and travel. A temporary loaner unit must be provided at no additional cost while an endoscope is undergoing repair. The contractor is responsible for all supervision, labor, parts, materials, tools, supplies, maintenance, ground transportation, and equipment services. All repairs must restore endoscopes to original Equipment Manufacturer (OEM) specifications, using new, OEM-approved replacement parts. Technicians must be OEM trained and certified, with documentation submitted with the offer. Services must comply with OEM specifications, NFPA 99 and 101, The Joint Commission standards, and HIPAA requirements. The Statement of Work (SOW) lists 17 specific endoscopes by make, model, serial number, and department.

Submission & Evaluation
Offerors must submit written proposals on an "all or none" basis. Quotes must include CAGE Code, DUNS, Tax Identification Number, FOB Point, Discount Terms, Delivery Date, a technical description, and past performance records. An acknowledgment of a 120-day acceptance period is required. All offerors must be registered in the System for Award Management (SAM). The Government will evaluate offers on a Best Value basis, considering Technical Capability, Past Performance, and Price. Technical Capability and Past Performance are considered significantly more important than Price. Technical Capability will be assessed based on Technical Approach, Personnel Qualifications, and Quality Control Plan. Past Performance will evaluate experience, timeliness, customer satisfaction, and downtime minimization. The Government reserves the right to award without discussions.
